摘 要:目的探究反复呼吸道感染(RRTI)患儿血清维生素、人胰岛素样生长因子1(IGF-1)及T淋巴细胞亚群水平变化及临床意义。方法选取2019年3月-2020年3月间于海南医学院第一附属医院儿科就诊的RRTI患儿120例作为试验组,另择同期于医院进行体检的健康儿童100名作为对照组。比较两组血清维生素、IGF-1及T淋巴细胞亚群水平,并采用Pearson相关性分析法分析血清维生素、IGF-1以及T淋巴细胞亚群水平与儿童RRTI的相关性。结果试验组血清维生素A、D、E、IGF-1及全血CD3^(+)T、CD4^(+)T、CD4^(+)T/CD8^(+)T水平均低于对照组(P<0.05);而全血CD8^(+)T水平高于对照组(P<0.05)。血清维生素A、D、E、IGF-1、CD3^(+)T、CD4^(+)T、CD4^(+)T/CD8^(+)T水平与儿童RRTI呈负相关(P<0.001),CD8^(+)T水平与儿童RRTI呈正相关(P<0.001)。结论血清维生素的缺乏和免疫功能紊乱与RRTI发病密切相关,临床可通过检测上述指标水平评价RRTI患儿病情,同时可补充维生素、改善免疫功能以降低RRTI发病频率。OBJECTIVE To investigate the changes of the levels of serum vitamin,insulin-like growth factor 1(IGF-1)and T lymphocyte subsets in children with recurrent respiratory tract infection(RRTI)and their clinical significance.METHODS One hundred and twenty RRTI children who visited the First Affiliated Hospital at Hainan Medical University between Mar 2019 and Mar 2020 were recruited as the observation group,and 100 healthy children under physical examination during the same period were in the control group.The levels of serum vitamin,IGF-1 and T lymphocyte subsets between the two groups were compared,and Pearson correlation analysis was used to analyze the correlation between the indexes and RRTI in children.RESULTS The levels of serum vitamin A,D,E,serum IGF-1,whole blood CD3^(+)T,CD4^(+)T,CD4^(+)T/CD8^(+)T in the observation group were significantly lower than those in the control group(P<0.05),while the level of whole blood CD8^(+)T in the observation group was significantly higher than that in the control group(P<0.05).The levels of serum vitamin A,D,E,IGF-1,whole blood CD3^(+)T,CD4^(+)T and CD4^(+)T/CD8^(+)T were negatively correlated with RRTI in children(P<0.001),and the level of CD8^(+)T was positively correlated with RRTI in children(P<0.001).CONCLUSION Serum vitamin deficiency and immune dysfunction were closely related to RRTI.The levels of serum vitamin A,D,E,IGF-1,whole blood CD3^(+)T,CD4^(+)T and CD4^(+)T/CD8^(+)T were negatively correlated with RRTI in children,and the level of CD8^(+)T was positively correlated with RRTI.Detection of the above index levels can evaluate the disease condition of RRTI children clinically,which can be the reference for the vitamin supplementation and immune function improvement,and to reduce the incidence frequency of RRTI.
