摘 要:目的:探讨CT关节造影与MRI关节造影对肩关节前脱位的诊断价值。方法:对36例(36个患肩)肩关节前脱位患者分别行CT造影与MRI造影,比较2种检查方法对下盂肱韧带损伤、前下盂唇损伤、骨与软骨损伤的检出率,并与肩关节镜或开放手术检查结果对照。结果:36例(36个患肩)中,下盂肱韧带损伤24个(CT造影检出14个,MRI造影检出22个);前下盂唇损伤32个(CT造影检出27个,MRI造影检出30个);骨与软骨损伤30个,其中骨折18个(CT造影检出18个,MRI造影检出8个),骨挫伤10个(CT造影未检出,MRI造影检出10个),软骨损伤5个(CT造影未检出,MRI造影检出5个)。对下盂肱韧带损伤,MRI造影的检出率优于CT造影(P<0.05);对前下盂唇损伤,CT造影与MRI造影检出率相似(P>0.05)。结论:对于肩关节前脱位的诊断,CT造影更利于发现骨折块,而MRI造影对观察骨挫伤及软骨损伤更有优势。Objective:To investigate the diagnostic value of CT arthrography and MRI arthrography in anterior dislocation of shoulder.Methods:CT arthrography and MRI arthrography examinations were performed on 36 patients due to anterior dislocation of shoulder.The detection rate of inferior glenohumeral ligament,anterior inferior labrum,bone and cartilage injuries by those two methods was calculated and compared with the results of arthroscopy or surgery.Results:A total of 36 patients with 36 affected shoulders included 24 cases with inferior glenohumeral ligament tearing(14 cases detected by CT arthrography and 22 cases by MRI arthrography);32 cases with anterior inferior labrum tearing(27 cases detected by CT arthrography and 30 cases by MRI arthrography);18 cases of fracture(18 detected by CT arthrography,8 by MRI arthrography),10 cases of bone contusion(no contusion detected by CT arthrography,10 by MRI arthrography),and 5 cases of cartilage injury(no cartilage injury detected by CT arthrography,5 by MRI arthrography).The detection rate of inferior glenohumeral ligament injury was higher for MRI arthrography than for CT arthrography(P<0.05).For anterior inferior labrum injury,the detection rate was similar for CT arthrography and MRI arthrography(P>0.05).Conclusions:CT arthrography is more helpful than MRI arthrography to detect fracture,while MRI arthrography could be more advantageous than CT arthrography to detect bone contusion and cartilage injury.
