加味当归补血汤治疗2型糖尿病肾病对机体微炎症状态、肾脏功能的影响

摘　　要：目的观察加味当归补血汤治疗2型糖尿病肾病对机体微炎症状态、肾脏功能的影响,为临床治疗提供参考。方法选取医院2016年11月—2019年6月收治的2型糖尿病肾病患者116例,根据治疗方法分为两组,每组58例。常规组给予常规对症治疗,中药组在常规组基础上给予加味当归补血汤治疗。记录两组治疗前后炎症因子、转化生长因子-β1(TGF-β1)、结缔组织生长因子(CTGF)、血管内皮生长因子(VEGF)、24 h尿蛋白定量(24hpro)、尿清蛋白排泄率(UAER)水平变化,并统计两组临床治疗总有效率和治疗期间不良反应情况。结果治疗后,中药组总有效率(84.48%,49/58)高于常规组(68.97%,40/58),不良反应率(8.62%,5/58)与常规组(6.70%,4/58)比较,差异不具有统计学意义(P>0.05)。治疗后,两组炎症因子、TGF-β1、CTGF、VEGF、24hpro、UAER水平较治疗前下降,且中药组高敏-C反应蛋白(hs-CRP)、单核细胞趋化蛋白-1(MCP-1)、可溶性细胞间黏附分子-1(sICAM-1)、白细胞介素-18(IL-18)、TGF-β1、CTGF、VEGF、24hpro、UAER水平低于常规组(P<0.05)。结论加味当归补血汤治疗2型糖尿病肾病可调节机体微炎症状态,保护肾功能,且不增加不良反应。Objective To observe the effect of Jiawei Danggui Buxue Decoction (加味当归补血汤) on microinflammation and renal function in type 2 diabetic nephropathy so as to provide reference for clinical treatment. Methods A total of 116 patients with type 2 diabetic nephropathy treated in our hospital from November 2016 to June 2019 were divided into two groups according to the treatment method, 58 cases in each group. The routine group was treated with routine symptomatic treatment, and the traditional Chinese medicine group was treated with Jiawei Danggui Buxue Decoction on the basis of the routine group. The changes of inflammatory factors, transforming growth factor-β1 (TGF-β1), connective tissue growth factor (CTGF), vascular endothelial growth factor (VEGF), 24 h urinary protein quantity (24 hpro) and urinary albumin excretion rate (UAER) were recorded before and after treatment, and the total effective rate of clinical treatment was counted. And adverse reaction conditions during and after treatment were compared. Results After treatment, the total effective rate (84.48%,49/58) of the traditional Chinese medicine group was higher than that of the conventional group (68.97%,40/58), and the adverse reaction rate (8.62%,5/58) was higher than that of the routine group (6.70%,4/58), and the difference was not statistically significant (P>0.05). After treatment, the levels of TGF-β1,CTGF,VEGF,24 hpro and UAER in the two groups were lower than those before treatment, and the levels of hs-CRP, MCP-1, sICAM-1, IL-18, TGF-β1,CTGF,VEGF,24 hpro and UAER in the traditional Chinese medicine group were lower than those in the routine group. Conclusion Jiawei Danggui Buxue Decoction can regulate the microinflammatory state of the body, protect renal function and do not increase adverse reactions in the treatment of type 2 diabetic nephropathy.
