摘  要:绝经后骨质疏松症(postmenopausal osteoporosis,PMOP)是一种与女性衰老、卵巢组织衰退有关的慢性代谢性骨病,以骨量降低,易发生骨折为其主要特征。成骨与脂肪细胞是骨髓微环境的重要组成部分,主要由骨髓间充质干细胞(bone marrow mesenchymal stem cells,BMSCs)分化形成,在生理状态下,BMSCs分化为成骨、成脂细胞呈动态平衡的特点,而绝经后骨质疏松症状态下,BMSCs成脂分化增强,成骨分化减弱。目前,较多研究集中于BMSCs成骨-成脂分化平衡对PMOP的影响,认为BMSCs过度成脂分化是PMOP发生、发展的病理影响因素。BMSCs分化为成脂细胞是骨髓脂肪的主要来源,大量研究发现在PMOP状态下骨量与骨髓脂肪呈负性相关,并且骨髓脂肪作为参与调节能量与内分泌功能的组织可能通过其分泌的脂肪因子、炎症因子,调节OPG/RANKL/RANK信号轴等直接或间接抑制成骨分化与骨形成,促进破骨分化与骨吸收,从而负向影响骨代谢。中药在调节成脂分化方面发挥重要作用,该文综述单味中药及其有效成分或其单体成分、复方中药调节成脂分化的最新研究进展,以期为绝经后骨质疏松症的科研及临床用药提供一定的依据和参考。Postmenopausal osteoporosis is a chronic metabolic bone disease related to female aging and ovarian tissue degeneration,its main characteristics are bone mass reduction and fracture susceptibility.Osteogenesis and adipocytes are important components of bone marrow microenvironment,mainly formed by differentiation of bone marrow mesenchymal stem cells(BMSCs). In physiological state,the differentiation of BMSCs into osteogenesis and adipocytes is dynamically balanced. While in postmenopausal osteoporosis,BMSCs adipogenic differentiation is enhanced and osteogenic differentiation is weakened. At present,many studies focus on the influence of BMSCs osteogenic-adipogenic differentiation balance on PMOP,and think that BMSCs excessive adipogenic differentiation is the pathological influencing facor of PMOP occurrence and development. The differentiation of BMSCs into adipocytes is the main source of bone marrow fat. A large number of studies have found that bone mass is negatively correlated with bone marrow fat in PMOP state,and bone marrow fat,as a tissue involved in regulating energy and endocrine function,may directly or indirectly inhibit osteogenic differentiation and bone formation,promote osteoclast differentiation and bone resorption,and thus negatively affect bone metabolism through its secretion of adipokines,inflammatory factors and OPG/RANKL/RANK signal axis. Traditional Chinese medicine plays an important role in regulating adipogenic differentiation. This paper reviews the latest research progress of regulating adipogenic differentiation by single traditional Chinese medicine and its effective components or monomer components and compound traditional Chinese medicine,in order to provide some basis and reference for scientific research and clinical medication of postmenopausal osteoporosis.
