摘　　要：目的探讨示指背侧与拇指掌背侧复合皮瓣修复拇指远节脱套样软组织缺损的疗效。方法回顾性分析42例拇指远节脱套样软组织缺损患者的临床资料,按照修复方案的不同分为观察组(18例)和对照组(24例)。观察组患者行示指背侧与拇指掌背侧复合皮瓣修复,对照组患者行前臂岛状皮瓣修复。比较2组患者皮瓣面积、远节指间关节运动幅度、痛觉满意度、触觉满意度及术后并发症发生率。结果2组患者远节指间关节运动幅度和术后并发症发生率比较差异无统计学意义(P>0.05);观察组患者皮瓣面积、痛觉满意度、触觉满意度均优于对照组,差异有统计学意义(P<0.05)。结论示指背侧与拇指掌背侧复合皮瓣修复拇指远节脱套样软组织缺损,患者手部感觉功能恢复更好,外观更为美观。Objective To investigate the curative effect of composite flap of dorsal side of index finger and palm-dorsal side of thumb on repairing the degloving soft tissue defect of thumb distal segment.Methods The clinical data of 42 patients with degloving soft tissue defect of thumb distal segment were retrospectively analyzed,and they were divided into the observation group(18 cases)and the control group(24 cases)according to the different repair plan.The patients in the observation group were repaired with the composite flap of dorsal side of index finger and palm-dorsal side of thumb,and the patients in the control group were repaired with the forearm island skin flap.The area of skin flap,distal interphalangeal joint motion amplitude,algesia satisfaction,tactile satisfaction and incidence of postoperative complications between the two groups were compared.Results There was no statistically significant difference in the distal interphalangeal joint motion amplitude and the incidence of postoperative complications between the two groups(P>0.05).The area of skin flap,algesia satisfaction and tactile satisfaction of the observation group were better than those of the control group,and the differences were statistically significant(P<0.05).Conclusion The composite flap of dorsal side of index finger and palm-dorsal side of thumb repairing the degloving soft tissue defect of thumb distal segment can achieve better recovery of the sensory function of patients’hand,and the appearance of hand is more beautiful.
