金脊鹿角汤结合针刺导热疗法治疗强直性脊柱炎(肾阳亏虚证)临床疗效

摘　　要：目的:探究金脊鹿角汤结合针刺导热疗法治疗强直性脊柱炎(肾阳亏虚证)临床疗效及其安全性。方法:符合纳入标准的135例强直性脊柱炎(肾阳亏虚证)患者,采取随机数字表将其分为观察组与对照组,对照组(67例)予以西医常规治疗,观察组(68例)予以西医常规治疗加金脊鹿角汤结合针刺导热疗法治疗,比较两组患者临床疗效、治疗前后中医症状(腰背及腰骶颈部疼痛、关节活动受限、晨僵、肢冷畏寒、口干口渴等)积分变化、Bath强直性脊柱炎功能指数(BASFI)、病情活动指数(BASDAI)评分、脊柱疼痛评分、全身疼痛评分、血沉(ESR)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)及不良反应情况。结果:观察组患者治疗总有效率(98.53%,67/68)高于对照组(88.06%,59/67)(P<0.05);治疗前,两组患者腰背及腰骶颈部疼痛、关节活动受限、晨僵、肢冷畏寒、口干口渴等中医症状积分、BASFI评分、BASDAI评分、脊柱疼痛评分、全身疼痛评分、ESR、TNF-α及CRP水平比较,没有统计学意义(P>0.05);治疗后各组患者腰背及腰骶颈部疼痛、关节活动受限、晨僵、肢冷畏寒、口干口渴等中医症状积分、BASFI评分、BASDAI评分、脊柱疼痛评分、全身疼痛评分、ESR、TNF-α及CRP水平均明显改善(P<0.05),且观察组优于对照组(P<0.05);观察组患者不良反应率(2.94%,2/68)低于对照组(11.94%,8/67)(P<0.05)。结论:金脊鹿角汤结合针刺导热疗法治疗强直性脊柱炎(肾阳亏虚证)效果显著,患者临床症状改善,脊柱功能恢复良好,疼痛减轻,可降低西药治疗带来的不良反应,安全可靠。Objective:To explore the clinical efficacy and safety of Jinji Lujiao Decoction combined with acupuncture thermal conduction therapy in the treatment of ankylosing spondylitis(AS)with Kidney-yang deficiency.Methods:135 cases of AS with Kidney-yang deficiency met the inclusion criteria were randomly divided into the observation group(n=68)and the control group(n=67).The control group was treated with conventional western medication;on which basis,the observation group was also treated with Jinji Lujiao Decoction combined with acupuncture thermal conduction therapy.Clinical efficacy,the scores of TCM symptoms(backache,lumbosacral pain,neck pain,limited joint activities,morning stiffness,cold limbs and aversion to cold,dry mouth,thirst),Bath Ankylosing spondylitis functional index(BASFI)and Bath Ankylosing spondylitis disease activity index(BASDAI),spinal pain and the whole body pain,the levels of erythrocyte sedimentation rate(ESR),tumor necrosis factor alpha(TNF-α)and c-reactive protein(CRP),as well as adverse reactions were compared between the two groups after the treatment.Results:The total effective rate of the observation group was 98.53%(67/68),which was significantly higher than 88.06%(59/67)of the control group(P<0.05).In terms of the scores of TCM symptoms,BASFI and BASDAI,spinal pain and the whole body pain,and in terms of the levels of ESR,TNF-αand CRP,there were no statistical differences between the two groups(P>0.05);they were all improved after the treatment in the two groups(P<0.05);the improvements in the observation group were more significant(P<0.05).The adverse reaction rate was 2.94%(2/68)in the observation group,which was significantly lower than 11.94%(8/67)in the control group(P<0.05).Conclusion:Jinji Lujiao Decoction combined with acupuncture thermal conduction therapy has a significant effect on AS with Kidney-yang deficiency.It can improve TCM symptoms and spinal function recovery,relieve pain and reduce the side effects induced by western medication as well.The therapy is safe and rel
