针刺翳风穴为主结合定痫汤治疗脑卒中后癫痫临床观察

摘　　要：目的观察针刺翳风穴为主与定痫汤治疗脑卒中后癫痫的临床疗效。方法将我院脑卒中后癫痫患者84例随机分为治疗组与对照组各42例。两组患者在抗血小板聚集、调脂、降压、降糖等卒中相关治疗基础上,对照组采用口服曲莱-奥卡西平片,治疗组在对照组治疗基础上针刺翳风穴结合定痫汤治疗。治疗结束后统计各组患者总有效率、脑电图异常情况、癫痫积分及血清白介素-6(IL-6)、白介素-1β(IL-1β)表达的变化,评价临床疗效的差异。结果治疗组总有效率为明显优于对照组(P<0.05);治疗后20h视频脑电图结果比较,治疗组在癫痫样放电、局限性等指标上显著优于对照组;治疗后1个月、3个月的癫痫积分,治疗组显著低于对照组(P<0.05);治疗后,治疗组血清IL-6、IL-1β表达显著低于对照组(P<0.05)。结论中医综合治疗脑卒中后癫痫,相比于单纯抗癫痫药治疗,可加强治疗效果,提高治疗水平,值得在临床中应用推广。Objective To observe the clinical efficacy of acupuncture at Yifeng point combined with Dingxian Decoction in treatment of post-stroke epilepsy.Methods A total of 84 patients with post-stroke epilepsy were randomly divided into treatment group and control group with 42 cases.On the basis of stroke-related treatments such as anti-platelet aggregation,lipid-lowering,blood pressure reduction,and hypoglycemic treatment,control group was treated with oral Trilay-oxcarbazepine Tablets,and treatment group received acupuncture at Yifeng point combined with Dingxian Decoction on the basis of control group.After treatment,the total effective rate,electroencephalogram(EEG) abnormalities,epilepsy scores and changes in serum interleukin-6(IL-6) and IL-1β expressions in each group were counted to evaluate the differences in clinical efficacy.Results The total effective rate of treatment group was significantly higher than control group(P<0.05);comparison of the results of video EEG 20 h after treatment showed that treatment group had significant epileptiform discharges,limitations and other indicators better than control group;comparing the epilepsy scores of the two groups at 1 month after treatment and 3 months after treatment,treatment group were significantly lower than control group(P<0.05);serum IL-6 and IL in treatment group after treatmen and the expression of IL-1β were significantly lower than control group(P<0.05).Conclusion The comprehensive treatment of post-stroke epilepsy with traditional Chinese medicine can enhance the therapeutic effect and improve the level of treatment compared with the treatment of antiepileptic drugs alone.It is worthy of clinical application and promotion.
