大柴胡汤辅助治疗慢性胆囊炎对炎症因子、β-EP的变化研究

摘　　要：目的研究大柴胡汤辅助治疗慢性胆囊炎对患者炎症因子、β-内啡肽(β-EP)的变化影响。方法选取医院2017年2月-2019年2月收治的慢性胆囊炎患者110例,采用随机数字法分为两组。对照组给予消炎利胆片治疗,观察组加用大柴胡汤治疗,分析两组患者治疗后的临床效果。结果观察组总有效率(90.91%,50/55)高于对照组(76.36%,42/55),6个月后复发率(3.64%,2/55)低于对照组(20.00%,11/55),差异具有统计学意义(P<0.05)。观察组患者右上腹痛缓解时间(3.25±0.57)d、右上腹压痛缓解时间(4.05±0.86)d、发热缓解时间(4.51±0.92)d短于对照组(P<0.05)。两组治疗前炎症因子、β-EP、瘦素(LEP)水平组间比较,差异不具有统计学意义(P>0.05)。观察组治疗后白细胞介素-2(IL-2)、白细胞介素-8(IL-8)水平高于对照组,白细胞介素-1β(IL-1β)、β-EP、LEP水平低于对照组(P<0.05)。结论大柴胡汤辅助治疗慢性胆囊炎可减轻炎症反应,降低β-EP、LEP的表达水平,缩短疼痛、发热时间,提高疗效。Objective To study the effect of Dacaihu Decoction(大柴胡汤)on the changes of inflammatory factors andβ-endorphin(β-EP)in patients with chronic cholecystitis.Methods A total of 110 patients with chronic cholecystitis treated in our hospital from February 2017 to February 2019 were randomly divided into two groups.The control group was treated with Xiaoyan Lidan Tablet and the observation group was treated with Dacaihu Decoction.The clinical effect of the two groups after treatment was analyzed.Results The total effective rate in the observation group(90.91%,50/55)was higher than that in the control group(76.36%,42/55).After 6 months,the recurrence rate(3.64%,2/55)was lower than that in the control group(20.00%,11/55)and the difference was statistically significant(P<0.05).The patients in the observation group’s right upper abdominal pain remission time[(3.25±0.57)d],right upper abdominal tenderness remission time[(4.05±0.86)d]and fever remission time[(4.51±0.92)d]were shorter than those of the control group(P<0.05).There was no significant difference in the levels of inflammatory factors,β-EP or leptin(LEP)between the two groups before treatment(P>0.05).The levels of IL-2 and IL-8 in the observation group were higher than those in the control group,while the levels of IL-1β,β-EP and LEP in the observation group were lower than those in the control group.Conclusion Dacaihu Decoction can reduce inflammatory reaction,decrease the expression levels ofβ-EP and LEP,shorten the time of pain and fever,and improve the curative effect.
