化瘀排石汤对泌尿系结石体外冲击波碎石术后疗效及血清相关激素、细胞因子的影响

摘　　要：目的分析化瘀排石汤对泌尿系结石患者体外冲击波碎石(extracorporeal shock wave lithotripsy,ESWL)术后疗效及血清相关激素及细胞因子的影响。方法选取医院2016年12月-2019年1月进行ESWL术的泌尿系结石患者98例,根据治疗方法不同分为两组。对照组术后给予常规西药对症治疗,观察组术后在对照组基础上给予化瘀排石汤治疗,分析两组患者治疗后的临床疗效。结果观察组结石排尽时间(7.08±2.97)d、肉眼血尿消失时间(6.42±1.97)d及腰痛减轻时间(4.42±1.92)d短于对照组(P<0.05)。两组术后12 h时表皮生长因子(EGF)、内皮素-1(ET-1)、肿瘤坏死因子-α(TNF-α)水平组间比较,差异不具有统计学意义(P>0.05)。观察组术后3 d时EGF、ET-1、TNF-α水平低于对照组(P<0.05)。两组术后12 h时相关激素指标水平组间比较,差异不具有统计学意义(P>0.05)。观察组术后7 d时生长抑素(SST)、超氧化物歧化酶(SOD)水平高于对照组,8-羟基脱酸鸟苷酸(8-OHdG)水平低于对照组(P<0.05)。观察组12个月无石率为95.92%(47/49),对照组为83.67%(41/49),观察组12个月无石率高于对照组(P<0.05)。结论化瘀排石汤可清除ESWL术后氧自由基,保护血管内皮细胞,减轻炎症反应,促进排石效率,降低结石复发。Objective The influence of Huayu Paishi Decoction(化瘀排石汤)on the postoperative effect of extracorporeal shock wave lithotripsy(ESWL)and serum related hormones and cytokines in patients with urinary calculi was analyzed.Methods Ninety-eight patients with urinary calculi who underwent ESWL in our hospital from December 2016 to January 2019 were divided into two groups according to the different treatment methods.The control group was treated with routine western medicine after operation,and the observation group was treated with Huayu Paishi Decoction on the basis of the control group.The clinical efficacy of the two groups after treatment was analyzed.Results The stone excretion time[(7.08±2.97)d],the disappearance time of hematuria[(6.42±1.97)d]and the relief time of low back pain[(4.42±1.92)d]in the observation group were shorter than those in the control group(P<0.05).There was no significant difference in the levels of EGF,ET-1 or TNF-αbetween the two groups 12 h after operation(P>0.05).The levels of EGF,ET-1 or TNF-αin the observation group were lower than those in the control group 3 days after operation,but there was no significant difference in the levels of related hormones between the two groups 12 h after operation(P>0.05).On the 7 th day after operation,the levels of somatostatin(SST)and SOD in the observation group were higher than those in the control group and the level of 8-hydroxydeoxyguanylate was higher than that in the control group.The level of 8-OHdG was lower than that of the control group(P<0.05).The stone-free rate was 95.92%(47/49)in the observation group and that was 83.67%(41/49)in the control group at 12 months.The stone-free rate in the observation group was higher than that in the control group at 12 months(P<0.05).Conclusion Huayu Paishi Decoction can clear oxygen free radicals,protect vascular endothelial cells,reduce inflammatory reaction,promote stone removal efficiency and reduce stone recurrence after ESWL.
