经腹和腹腔镜全子宫切除术对患者盆底功能的影响分析

摘　　要：目的分析经腹和腹腔镜全子宫切除术对患者盆底功能的影响。方法180例行全子宫切除术的患者,以随机数字表法分为观察A组及观察B组,各90例。观察A组行经腹全子宫切除术,观察B组行腹腔镜全子宫切除术。比较两组患者盆底肌纤维肌力及盆底功能障碍情况。结果术前,两组盆底肌Ⅰ类肌纤维肌力、Ⅱ类肌纤维肌力分级比较,差异无统计学意义(P>0.05);术后,两组盆底肌Ⅰ类肌纤维肌力、Ⅱ类肌纤维肌力分级均低于术前,但观察B组术后肌Ⅰ类肌纤维肌力、Ⅱ类肌纤维肌力分级高于观察A组,差异有统计学意义(P<0.05)。观察B组盆底功能障碍总发生率3.33%,低于观察A组的18.89%,差异有统计学意义(P<0.05)。结论经腹全子宫切除术与腹腔镜全子宫切除术对患者盆底功能皆存在一定影响,腹腔镜全子宫切除术对盆底纤维肌力影响小,且盆底功能障碍发生率较低,临床可采用腹腔镜全子宫切除术,减轻对患者盆底功能的影响。Objective To analyze the effect of transabdominal and laparoscopic hysterectomy on pelvic floor function.Methods A total of 180 patients undergoing total hysterectomy were divided into observation group A and observation group B according to random numerical table,with 90 cases in each group.Observation group A was treated with transabdominal hysterectomy,and observation group B was treated with laparoscopic hysterectomy.The pelvic floor muscle fiber strength and pelvic floor dysfunction were compared between the two groups.Results Before operation,there was no statistically significant difference in gradeⅠandⅡpelvic floor muscle fiber strength between the two groups(P>0.05).After operation,the gradeⅠandⅡpelvic floor muscle fiber strength of the two groups were lower than those before operation,but the gradeⅠandⅡpelvic floor muscle fiber strength of observation group B was higher than that of observation group A,and the difference was statistically significant(P<0.05).The total incidence of pelvic floor dysfunction 3.33%of observation group B was lower than 18.89%of observation group A,and the difference was statistically significant(P<0.05).Conclusion Transabdominal hysterectomy and laparoscopic hysterectomy have some influence on pelvic floor function.Laparoscopic hysterectomy has little effect on pelvic floor muscle strength,and the incidence of pelvic floor dysfunction is low,laparoscopic hysterectomy can be used to reduce the impact on pelvic floor function.
