16例瘢痕子宫再次足月妊娠阴道试产分析

摘　　要：目的探讨瘢痕子宫再次足月妊娠阴道试产的效果。方法选取16例瘢痕子宫再次足月妊娠行阴道试产的产妇为观察组,另选取同期非瘢痕子宫初次足月妊娠阴道试产的产妇16例为对照组。比较两组产妇的分娩方式与分娩结果;两组阴道试产成功产妇的产程时间、新生儿Apgar评分、住院时间。结果观察组产妇中,10例实施阴道试产,6例实施剖宫产。阴道试产成功10例,成功率为62.5%(10/16)。对照组产妇均实施阴道试产,阴道试产成功14例,成功率为87.5%(14/16)。两组产妇的阴道试产成功率比较,差异无统计学意义(χ^(2)=2.667,P=0.102>0.05)。两组阴道试产成功产妇的产程时间、新生儿Apgar评分、住院时间比较,差异均无统计学意义(P>0.05)。结论瘢痕子宫再次足月妊娠是可以行阴道试产分娩的,如有试产适应证的,可以在监护下行阴道试产。Objective To discuss the effect of vaginal trial production of full-term pregnancy with scarred uterus.Methods There were 16 cases of vaginal trial production of full-term pregnancy with scarred uterus selected as the observation group,and 16 cases of vaginal trial production of first-term pregnancy with nonscarred uterine pregnancy as the control group.Comparison was made on delivery mode and results between the two groups,and labor time,neonatal Apgar score,hospitalization time of the women with successful vaginal trial production between the two groups.Results In the observation group,10 cases performed vaginal trial production and 6 cases performed cesarean section.There were 10 successful cases,with success rate of vaginal trial production as 62.5%(10/16).In the control group,all women received vaginal trial production,and 14 cases were successful,with success rate of vaginal trial production as 87.5%(14/16).There was no statistically significant difference in success rate of vaginal trial production between the two groups(χ^(2)=2.667,P=0.102>0.05).There was no statistically significant difference in labor time,neonatal Apgar score and hospitalization time of the women with successful vaginal trial production between the two groups(P>0.05).Conclusion Vaginal trial production can be performed in full-term pregnancy with scarred uterus.If there are indications for trial labor,vaginal trial production can be performed under supervision.
