眩晕引颈试验在颈性眩晕诊断中的运用研究

摘　　要：目的:通过观察自研眩晕引颈试验在临床鉴别诊断颈性眩晕与良性阵发性位置性眩晕(BPPV)时的效果差异,研发一种具有特异性的颈性眩晕诊断方法。方法:收集2017年12月—2019年7月江西中医药大学附属医院神经内科门诊和住院部符合颈性眩晕或BPPV常规诊断标准的眩晕患者各50例。眩晕引颈试验前两组患者行眩晕残障量表评分(DHI),结果显示两组眩晕程度无显著性差异。再对试验前后两组患者进行视觉模拟评分(VAS),根据患者在试验中的反应及VAS评分判定诊断结果,对评分进行统计学分析。结果:(1)试验前后颈性眩晕组VAS评分及试验后两组VAS评分有显著差异(P<0.01)。(2)眩晕引颈试验与常规诊断方法对眩晕患者的诊断具有可比性(P>0.05)。结论:自研眩晕引颈试验作为一种简便的体检方法,在颈性眩晕的诊断和鉴别诊断中具有较高的临床价值。作为颈性眩晕的一种诊断及鉴别诊断试验,适于在临床推广,应用前景较好。Objective:To study the difference effect of self-developed vertigo pull-neck test in clinical differentiation diagnosis of cervical vertigo and benign paroxysmal positional vertigo(BPPV),and to develop a specific diagnostic method for cervical vertigo.Methods:A total of 100 vertigo patients who met the routine diagnostic criteria of cervical vertigo and BPPV in the neurology outpatient department or inpatient department of the Affiliated Hospital of Jiangxi University of Chinese Medicine from December 2017 to July 2019 were collected,including 50 patients in the cervical vertigo group and 50 patients in the BPPV group.The vertigo disability scale(DHI)was scored in the two groups before the vertigo pull-neck test,and the results showed no significant difference in vertigo degree between the two groups.Then visual analogue scale(VAS)was performed for the two groups before and after the experiment,and the diagnostic results were determined according to the patients'response in the experiment and VAS score,and the scores were statistically analyzed.Results:(1)The VAS scores of cervical vertigo group before and after the experiment and the two groups after the experiment(P<0.01)were significantly different.⑵Thevertigo pull-neck test was comparable with conventional diagnostic methods in the diagnosis of vertigo patients(P>0.05).Conclusion:Asa simple physical examination method,self-developed of vertigo pull-neck test has high clinical value in the diagnosis and differential diagnosis of cervical vertigo.As a diagnostic and differential diagnosis test of cervical vertigo,it is suitable for clinical application and has a good application prospect.
