经皮椎体后凸成形术治疗骨质疏松性椎体压缩骨折围手术期并发症分析

摘　　要：目的回顾性分析经皮椎体后凸成形术(PKP)治疗骨质疏松性椎体压缩骨折(OVCFs)术中、术后并发症的发生情况及原因,并探讨预防措施。方法选取2017年7月~2019年12月海南省人民医院采用PKP治疗OVCFs患者156例(236个椎体),总结并发症的发生情况,分析并发症的发生原因。结果156例患者中发生并发症41例(26.3%),其中术中并发症38例(24.4%),包括骨水泥渗漏35例、穿刺入椎管1例、球囊破裂1例、骨水泥毒性1例;术后并发症16例(10.3%),包括邻椎骨折15例、骨水泥脱落1例。发生并发症的41例患者中出现1个并发症29例,出现2个并发症11例,出现3个并发症1例。结论PKP治疗OVCFs常见的并发症包括骨水泥渗漏、穿刺入椎管、球囊破裂、术后邻椎骨折等,术中及术后应采取相应的防范措施,预防或减少并发症的发生。Objective To retrospectively analyze the occurrence and causes of complications during and after percutaneous kyphoplasty(PKP)for osteoporotic vertebral compression fractures(OVCFs).Methods From July 2017 to December 2019,156 patients with OVCFs(including 236 vertebrae)were treated with PKP.The occurrence and the causes of complications were analyzed.Results Among 156 cases,there were 41 cases(26.3%)with complications.There were 38 case(24.4%)with intraoperative complications among them,including bone cement leakage in 35 cases,perforation into the spinal canal in 1 case,balloon rupture in 1 case,and toxicity of bone cement in 1 case.There were 16 cases(10.3%)with postoperative complications,including 15 cases with adjacent vertebral fractures and 1 case with cement shedding.Among 41 patients with complications,there were 29 patients with 1 complication,11 patients with 2 complications and 1 patient with 3 complications.Conclusion The common complications of PKP in the treatment of OVCFs include bone cement leakage,perforation into the spinal canal,balloon rupture and adjacent vertebral fracture.
