摘　　要：目的:观察电针联合神经肌肉控制训练治疗老年肩袖损伤的效果。方法:选取2017年10月至2019年10月于福建省福清市医院康复科门诊就诊的60例≥60岁慢性肩袖损伤患者随机分为治疗和对照组各30例。治疗组先运用电针治疗,再根据疼痛VAS评分划分轻度、中度、重度三个阶段进行神经肌肉控制训练,对照组采取Mulligan动态关节松动术、物理因子治疗及常规医疗体操训练。两组患者连续接受2个疗程康复治疗,4周为1个疗程。治疗前、治疗后第4、8周观察患者Constant-Murley肩关节功能评分(CMS)、视觉疼痛量表(VAS)和关节活动度(ROM)变化。结果:60例患者全部进入结果分析。治疗后第4、8周患者肩关节疼痛、ADL、活动度、肌力和Constant总分均较治疗前显著改善,且治疗组明显优于对照组(P<0.05)。治疗后第4、8周两组患者VAS评分明显低于治疗前,且治疗组VAS评分明显低于对照组(P<0.05)。治疗后第4、8周两组患者肩关节前屈、外展、体侧外旋活动度数均明显高于治疗前,且治疗组明显优于对照组(P<0.05)。结论:电针联合神经肌肉控制训练治疗老年肩袖损伤可缓解疼痛,提高关节活动度,显著改善患者肩关节功能。Objective:To investigate the efficacy of electroacupuncture combined with neuromuscular controll training on rotator cuff tears in elderly patients.Methods:60 patients aged over 60 years old with chronic rotator cuff tears in Fujian Province Fuqing City Hospital from October 2017 to October 2019 were randomly divided into treatment group and control group,30 cases in each group.The treatment group was treated with electroacupuncture,then combined with neuromuscular controll training according to the classification of mild,moderate and severe stages based on pain VAS score.The control group was treated with Mulligan dynamic joint mobilization,physical factor therapy and routine medical gymnastics training.Two groups of patients received 2 courses of treatment continuously,4 weeks for one therapeutic course.Both groups were assessed with Constant-Murley shoulder function score(CMS),visual analogy pain scale(VAS)and range of motion(ROM)before treatment and at the 4 th and 8 th week after treatment.Results:Results:All 60 patients were involved in the final analysis.At the 4 th and 8 th week after treatment,Shoulder pain,ADL,motion,muscle strength and total Constant score in both groups were significantly improved,and the treatment group was significantly superior to the control group(P<0.05).The VAS score of the patients in the 4 th and 8 th weeks after treatment decreased significantly,and the VAS score in the treatment group was significantly lower than that in the control group(P<0.05).At the 4 th and 8 th weeks after treatment,the degree of shoulder flexion,abduction and lateral rotation were significantly higher than those before treatment,and the treatment group was significantly superior to the control group(P<0.05).Conclusion:Electroacupuncture combined with neuromuscular control training can relieve pain,improve shoulder joint function and mobility significantly in elderly patients with rotator cuff tears.
