摘  要:目的探讨鸟苷酸结合蛋白5(GBP5)、miR-223在肺结核外周血单个核细胞中的表达及其临床意义。方法选取2018年9月-2019年9月广元市第一人民医院感染科收治的98例肺结核患者为肺结核组,另选取同期广元市第一人民医院呼吸科年纪相仿的肺炎患者98例为肺炎组。收集所有患者的性别、年龄、主诉等临床一般资料,于入院后次日清晨,采用全自动血球仪检测白细胞(WBC);全自动快速血沉分析仪检测红细胞沉降率(ESR),qRT-PCR技术检测GBP5、miR-223表达量。采用多因素Logistic回归分析影响肺结核发生的因素,ROC曲线分析GBP5、miR-223的诊断性能。结果两组患者性别、年龄、合并疾病(高血压、糖尿病、高血脂)、吸烟史及饮酒史、咳嗽等方面比较差异无统计学意义(P>0.05),但肺结核组的咳痰、发热发生率显著低于肺炎组,差异有统计学意义(P<0.05)。与肺炎组患者比较,肺结核组患者的ESR、GBP5表达量显著升高,miR-223表达量显著降低,差异有统计学意义(P<0.05)。GBP5增加(OR=2.016,P=0.001)、miR-223降低(OR=0.444,P=0.025)均是影响肺结核发生的独立危险因素。ROC曲线结果显示,GBP5的AUC、灵敏度和特异性分别为0.875、79.59%、85.71%,miR-223分别为0.944、81.63%、93.88%;经比较,miR-223的诊断价值高于GBP5,差异有统计学意义(P<0.05)。结论相较于肺炎患者,肺结核患者外周血单个核细胞中GBP5表达增多、miR-223表达降低,GBP5、miR-223均可用于肺结核的鉴别,且miR-223诊断价值更高。Objective To explore expression levels of guanylate-binding protein 5(GBP5)and miR-223 in peripheral blood mononuclear cells of tuberculosis and their clinical significance.Methods During the period from September 2018 to September 2019,98 tuberculosis patients who were admitted to the First People’s Hospital of Guangyuan were enrolled as tuberculosis group.A total of 98 pneumonia patients with similar age who underwent physical examination in the hospital during the same period were enrolled as pneumonia group.The clinical general data such as gender,age and chief complaints of all patients were collected.In the early morning of the next day after admission,white blood cell(WBC)was detected by full-automatic hemocytometer.Erythrocyte sedimentation rate(ESR)was detected by full-automatic rapid erythrocyte sedimentation analyzer.The expression quantities of GBP5 and miR-223 were detected by qRT-PCR.Multivariate logistic regression analysis was applied to analyze factors influencing occurrence of tuberculosis.ROC curves were applied to analyze diagnostic efficiency of GBP5 and miR-223.Results There were no significant differences in gender,age,complications(hypertension,diabetes,hyperlipidemia),smoking and drinking history,and cough between the two groups(P>0.05).The incidence of expectoration and fever in tuberculosis group was significantly lower than that in pneumonia group(P<0.05).Compared with pneumonia group,ESR and expression level of GBP5 were significantly increased,while expression level of miR-223 was significantly decreased in tuberculosis group(P<0.05).Increase of GBP5 level(OR=2.016,P=0.001)and decrease of miR-223 level(OR=0.444,P=0.025)were independent risk factors influencing occurrence of tuberculosis.ROC curves results showed that AUC,sensitivity and specificity of GBP5 and miR-223 were 0.875,79.59%,85.71%and 0.944,81.63%,93.88%,respectively.After comparison,diagnostic value of miR-223 was higher than that of GBP5(P<0.05).Conclusions Compared with pneumonia patients,expression level of GBP5 was
