中医针刺治疗癌性疼痛的研究进展

摘　　要：在面临癌性疼痛作为癌症患者最感痛苦的症状下,缓解癌性疼痛,减少患者痛苦成为重要的临床工作之一。然而现有的三阶梯止痛法并未全部缓解癌症患者疼痛症状,且药物治疗癌性疼痛所带来的不良反应也不可忽至。针刺治疗癌性疼痛,作为一种有效、便捷、不良反应少的方法于临床发挥着其特有的疗效。针刺治疗癌性疼痛的方法现已经被应用于临床之中,然而回顾各个临床试验可以发现,针对针刺选穴处方的制定不尽相同。并且少有文章能明确得阐述选穴依据。文章回顾了近年有关针灸治疗癌性疼痛的临床研究,虽各个临床试验可以证明针刺对于治疗癌性疼痛有疗效,但实验所选用的针刺方法与针刺穴位未有明确规定。对于实验中所用到不同针刺方法加以总结,简述其产生疗效的机制,使得针刺治疗癌性疼痛有效这一理论更加有理可循。并对不同的选穴方法加以论述,阐释确定针刺处方的依据。并且结合伴有癌性疼痛的患者的相关调查结果,提出了选穴原则应结合患者证型而制定这一理念。通过对临床试验的针刺方法与选穴方法的回顾,提出今后对于针刺治疗癌性疼痛的相关选穴方法应制定更加系统的方案,从而使得针刺治疗癌性疼痛的选穴方案更加有理可依,提高针刺疗效的同时为临床治疗提供准确系统的选穴方案。In the face of cancer pain as the most painful symptom of cancer patients,it has become one of the important clinical work to relieve cancer pain and reduce the pain of patients.However,the existing three-step analgesic method has not completely alleviated the pain symptoms of cancer patients,and the adverse reactions caused by drug treatment of cancer pain can not be ignored.Acupuncture treatment of cancer pain,as an effective and convenient method with few side effects in clinic,plays its unique curative effect.Acupuncture treatment of cancer pain has been applied in clinical practice.However,reviewing the clinical trials,it can be found that the acupuncture prescription is not the same.And few articles can clearly explain the basis of acupoint selection.This paper reviewed the recent clinical studies on the treatment of cancer pain by acupuncture and moxibustion.The results showed that acupuncture was effective on treating cancer pain in various clinical trials,but the acupuncture methods and acupuncture points selected in the experiment were not clearly defined.The author summarized the different acupuncture methods used in the experiment,and briefly described the mechanism of its curative effect,which made the theory of acupuncture effective in the treatment of cancer pain more reasonable.Different methods of acupoint selection were discussed,and the basis for determining acupuncture prescription was explained.Combined with the relevant investigation results of patients with cancer pain,the principle of acupoint selection should be combined with the syndrome type of patients.Based on the review of acupuncture methods and acupoint selection methods in clinical trials,it is suggested that a more systematic scheme should be developed for acupoint selection methods related to acupuncture treatment of cancer pain in the future.Therefore,the acupoint selection scheme of acupuncture treatment for cancer pain is more reasonable,which can improve the acupuncture effect and provide accurate and systematic point select
