参芪地黄汤对气阴两虚型早期糖尿病肾病血清炎症因子、肾功能及微循环的影响

摘　　要：目的探讨参芪地黄汤在气阴两虚型早期糖尿病肾病患者中的治疗效果。方法将2018年1月—2020年1月收治的86例气阴两虚型早期糖尿病肾病患者纳入本次的研究,按照随机数字表法分为对照组与观察组各43例。对照组在常规治疗基础上加用厄贝沙坦,0.15 g/次,1次/d,连续治疗8周,观察组在对照组基础上加用参芪地黄汤,水煎400 m L,1剂/d,分两次服用,连续治疗8周。比较两者治疗前后中医证候积分、血清炎症因子、肾功能。结果治疗前,两组中医证候积分比较,差异无统计学意义(P> 0.05),具有可比性,治疗后,两组中医证候积分均降低,观察组低于对照组,差异有统计学意义(P <0.05)。治疗前,两组IL-6、IL-8、TNF-α、CRP等比较,差异无统计学意义(P> 0.05),具有可比性,治疗后,观察组患者的IL-6、IL-8、TNF-α、CRP等血清炎症因子指标水平均明显低于对照组,差异有统计学意义意义(P <0.05)。治疗前,两组β2-MG、Cys-C、尿m-ALB比较,差异无统计学意义(P>0.05),具有可比性,治疗后,观察组β2-MG、Cys-C、尿m-ALB等肾功能指标均明显低于对照组患者(P <0.05)。结论参芪地黄汤在气阴两虚型早期糖尿病肾病患者中具有确切的治疗效果,可有效降低患者的中医证候积分,改善血清炎症因子、改善肾功能,具有良好的应用效果,值得推广。Objective To explore the therapeutic effect of Shenqi Dihuang decoction(参芪地黄汤) in early diabetic nephropathy patients with deficiency of both qi and Yin.Methods From January 2018 to January 2020,86 cases of early diabetic nephropathy with deficiency of both qi and Yin were included in the study.According to the random number table,they were divided into control group and observation group with 43 cases each.In the control group,irbesartan was added on the basis of routine treatment,0.15 g/time,once a day,for 8 weeks.In the observation group,Shenqi Dihuang decoction was added on the basis of the control group,400 ml was decoction,1 dose/day,twice,for 8 weeks.Before and after treatment,TCM symptom scores,serum inflammatory factors and renal function were compared.Results Before the treatment,there was no significant difference between the two groups(P > 0.05),which was comparable.After the treatment,the scores of the two groups were reduced,the observation group was lower than the control group,the difference was statistically significant(P <0.05).Before treatment,there was no significant difference in IL-6,IL-8,TNF-α,CRP between the two groups(P >0.05),which was comparable.After treatment,the levels of IL-6,IL-8,TNF-α,CRP and other serum inflammatory factors in the observation group were significantly lower than those in the control group,the difference was statistically significant(P < 0.05).Before treatment,there was no significant difference in β 2-mg,Cys-C and urinary m-alb between the two groups(P > 0.05),which was comparable.After treatment,the renal function indexes such as β 2-mg,Cys-C and urinary m-alb in the observation group were significantly lower than those in the control group(P < 0.05).Conclusion Shenqi Dihuang decoction has definite therapeutic effect in early diabetic nephropathy patients with deficiency of both qi and Yin.It can effectively reduce the syndrome score of patients,improve serum inflammatory factors,improve renal function,and has good application effect,which is worth
