益气养阴汤联合中药封包热敷治疗气阴两虚兼瘀型慢性心力衰竭稳定期的临床观察

摘　　要：目的观察益气养阴汤联合中药封包热敷治疗气阴两虚兼瘀型慢性心力衰竭稳定期患者的临床疗效,以期为临床治疗提供参考。方法选取医院2019年1月—2019年12月门诊诊治的慢性心力衰竭稳定期60例患者,采用随机数字表法均分为两组,观察组和对照组。对照组按照指南相关标准给予基础药物治疗,对照组则采用益气养阴汤联合中药封包热敷治疗,均连续治疗3个月。观察治疗前后临床疗效、中医症状评分及心功能变化情况,对比生活质量评分,统计不良反应发生情况。结果观察组中医证候总有效率90.00%(27/30)明显高于对照组66.77%(20/30),且观察组心功能分级总有效率83.33%(25/30)明显高于对照组60.00%(18/30),差异有统计学意义(P <0.05);治疗前两组心悸、气短/喘息、乏力、盗汗、五心烦热、舌质暗红、脉细数无力中医症状评分上无明差异(P>0.05);经治疗,两组上述症状评分均明显降低,但观察组评分明显低于对照组,差异有统计学意义(P <0.05);经治疗,观察组心功能LVEE、LVESD、LVESV指标改善情况明显优于对照组,差异有统计学意义(P <0.05);观察组血清NT-Pro BNP水平明显降低,且低于对照组,观察组6 min步行实验明显长于对照组,差异有统计学意义(P <0.05);观察组生活质量评分明显优于对照组,差异有统计学意义(P <0.05);治疗期间,观察组不良情况发生率6.67%(2/30),对照组不良反应发生率16.67%(5/30),两组不良反应无统计学意义(P> 0.05)。结论对气阴两虚兼瘀型慢性心力衰竭稳定期患者采用益气养阴汤联合中药封包热敷治疗可发挥中医药多靶点作用,改善患者临床症状及心功能,提高生活质量,可为日后治疗提供参考。Objective To observe the clinical efficacy of Yiqi Yangyin decoction(益气养阴汤) combined with traditional Chinese medicine package hot compress in the treatment of chronic heart failure in patients with both Qi and Yin deficiency and stasis,with a view to providing reference for clinical treatment.Methods Sixty patients with stable chronic heart failure who were admitted to the Department of Cardiology in our outpatient clinic from January 2019 to December 2019 were selected and divided into two groups,the observation group and the control group.The control group was given basic drug treatment in accordance with the relevant standards of the guidelines,and the observation group was treated with Yiqi Yangyin Decoction combined with Chinese medicine package hot compress,both of which were treated for 3 months.We observed the clinical efficacy,TCM symptom scores and heart function changes before and after treatment,compared the quality of life scores and counted the occurrence of adverse reactions.Results The total effective rate of TCM syndromes in the observation group was 90.00%(27/30),which was significantly higher than that in the control group 66.67%(20/30),and the total effective rate of heart function classification in the observation group was 83.33%(25/30),which was significantly higher than that in the control group 60.00%(18/30).The difference was statistically significant(P < 0.05).Before treatment,palpitation,shortness of breath/wheezing,fatigue,night sweats,five upset fever,dark red tongue,weak pulse countand no significant difference in TCM symptom scores in thetwo groups were not significant(P > 0.05).After treatment,the above symptom scores in both groups were significantly reduced.But the observation groups score was significantly lower than that of the control group and the difference was statistically significant(P < 0.05).After treatment,the improvements of LVEE,LVESD and LVESV of the observation group were significantly better than those of the control group and the difference was sta
