甲地孕酮联合针灸改善癌性厌食的临床研究

摘　　要：目的观察甲地孕酮联合针灸疗法治疗癌性厌食的疗效。方法随机选取柳州市柳铁中心医院肿瘤科2018年1月—2020年1月连续入院诊断为癌性厌食的中晚期肿瘤患者99例,根据入院日期为偶数和奇数分为2组,非联合组52例单用甲地孕酮口服治疗,联合组47例在非联合组基础上加用针灸治疗,2组均以2周为1个疗程,治疗2个疗程。观察2组治疗后体质量、血前白蛋白、白蛋白水平变化及食欲改善情况。结果联合组与非联合组体重有效分别为42例(89.4%)、32例(61.5%),食欲改善分别为31例(66.0%)、20例(38.5%),联合组体重有效率及食欲改善率均高于非联合组(P均<0.05)。联合组治疗后前血白蛋白、白蛋白水平均明显高于治疗前及非联合组(P均<0.05)。结论甲地孕酮联合针灸疗法可明显改善中晚期肿瘤患者癌性厌食症状及营养状况。Objective It is to observe the efficacy of megestrol combined with acupuncture in the treatment of cancer-induced anorexia.Methods Ninety-nine patients with advanced cancer who were diagnosed with cancerous anorexia were randomly selected from the Oncology Department of Liuzhou Liutie Central Hospital from January 2018 to January 2020,and were divided into 2 groups according to the date of admission as even and odd numbers,the non-combined group including 52 cases were treated with megestrol alone orally,and 47 cases in the combined group were treated with acupuncture on the basis of the non-combined group.2 weeks as a course of treatment and both groups were treated for 2 courses of treatment.The changes in body weight,blood prealbumin and albumin levels and the improvement of appetite in the two groups after treatment were observed.Results The effective rate in the body curative effect of the combined group and the non-combined group were 89.4%(42/47)and 61.5%(32/52),respectively,while the improving rate of appetite were 66.0%(31/47)and 38.5%(20/52),respectively.The effective rate of body weight and the improving rate of the combined group were higher than those of the non-combined group(P<0.05).The levels of pre-albumin and albumin of the combined group after treatment were significantly higher than those of the pre-treatment and non-combined groups(P<0.05).Conclusion Megestrol combined with acupuncture therapy can significantly improve the symptoms and nutritional status of cancer-related anorexia in patients with advanced tumors.
