摘　　要：慢性病作为威胁我国居民健康的头号杀手,其防控刻不容缓。文章通过对海宁市医共体建设工作实践的总结得出,只有真正使得县级医院与基层卫生院、社区服务站紧密有效合作,形成利益共同体和责任共同体,才能利于慢病管理服务,把工作落到实处,取得积极成效。The prevention and control of chronic diseases are critical as they are taken as the number one killer threatening the health of Chinese residents.This paper concludes,based on the practice of medical community in recent years,that only by realizing an effective cooperation among county hospitals,primary health centers and community service stations and forming a community of interests and a community of responsibilities can we benefit the management of chronic diseases,implement the work and achieve positive results.
