摘  要:目的:以小动物磁共振成像技术为研究手段,探寻胃俞募穴治疗对脑岛味觉皮层损毁功能性消化不良大鼠的全脑功能连接影响,深入分析胃俞募配穴治疗功能性消化不良(Functional Dyspepsia,FD)的中枢响应特点。方法:将60只雄性SD乳鼠随机分为手术针刺组、手术组、假手术组。采用碘乙酰胺灌胃、夹尾应激法,结合不规则喂养,分阶段制作FD模型。大鼠7周龄时,行脑岛味觉皮层损毁手术,手术针刺组与手术组以鹅膏蕈氨酸溶液注射,假手术组以磷酸缓冲盐溶液注射。FD造模成功后,手术针刺组以胃俞募配穴针刺10 d。针刺结束后,随机选取各组6只大鼠进行静息态fMRI扫描并进行全脑功能连接分析。结果:胃俞募配穴干预后,手术针刺组相较手术组,嗅结节与脑岛、海马与隔核连接减弱;嗅结节与缰核、丘脑,听觉皮层与梨状皮层、黑质与臂核连接增强。结论:胃俞募穴对FD大鼠的边缘系统、嗅觉皮层、基底神经节等与情绪、认知、内脏感觉调控相关的脑功能系统具有一定的调节作用,可能是治疗FD中枢响应机制之一。Objective:Based on the small animal fMRI technology,the study focuses on the influence of stomach back-Shu and frontal-Mu points on the functional connectivity of whole brain in Functional dyspepsia(FD)rats with injured gustatory cortex ofinsula.The central response mechanism of acupuncture at the stomach back-Shu and frontal-mu points for treating FD was further analyzed.Methods:Sixty male Sprague-dawley rats were randomly divided into acupuncture group,model group and sham group.Iodide acetamide sucrose solution feeding method and tail pinch fighting method with irregular feeding method were adopted in each group to build the FD model.The surgery to injure rats’gustatory cortex of insula was performed at the age of 7 weeks.The acupuncture group and model group were injected with ibotenic acid injection while sham group was given phosphate buffer salt solution injection.During the intervention,acupuncture group was treated with stomach back-Shu and frontal-Mu points for 10 days.Six rats in each group were randomly selected for resting stage fMRI scanning with whole-brain functional connectivity analysis.Results:After intervention,compared with the model group,reduced connections were shown in those brain regions of acupuncture group,including olfactory nodules and insula,hippocampus and septal nuclei.The brain regions which showed enhanced connections were olfactory bulb and habenula,thalamus,auditory cortex and piriform cortex,substantia nigra and parabrachial nuclei.Conclusion:Stomach back-Shu and frontal-Mu points can regulate the olfactory cortex,limbic system,basal ganglia and other brain systems closely related to emotion,cognition and visceral sensation in FD rats.It may be one of the central response mechanisms of stomach back-Shu and frontal-Mu points for treating FD.
