授权理论应用对初产妇母婴结局及心理状态的影响

摘　　要：目的研究授权理论应用对初产妇母婴结局及心理状态的影响。方法以2018年6月至2019年6月在深圳市宝安区妇幼保健院产检的200例初产妇为干预实施对象,随机数字表法分为授权管理组(应用授权理论为框架的管理措施)和常规管理组(使用常规管理措施),每组各100例。比较方案实施后两组初产妇在分娩计划、母婴结局及焦虑得分之间的差异。结果方案实施后,授权管理组和常规管理组在自然分娩率[(93.8%vs.83.5%),P<0.05]、产后2 h出血率[(2.1%vs.7.2%),P<0.05]及焦虑得分[(54.51±4.21)vs.(63.49±3.98),P<0.05]均显示出有统计学差异,新生儿窒息率未显示出统计学差异(P>0.05)。结论以授权理论为框架的授权管理方案是一种有效的孕期管理措施,应用至初产妇的孕期管理中可有效改善其焦虑情绪,减少母体分娩时的不良风险,并能够改变初产妇的分娩计划,提高自然分娩率,值得临床借鉴。Objective To study the effect of empowerment theory on outcomes and mental states of primiparas.Methods From June 2018 to June 2019,200 cases of primiparas delivered in the Maternal and Child Health Hospital of Bao'an district were selected and divided into intervention group(management measures based on empowerment theory)and control group(using conventional management measures)by the method of random number table.The maternal and infant outcomes and anxiety scores between the two groups were compared.Results After the implementation of the program,there were statistical differences between the empowerment management group and the conventional management group in the rate of natural delivery(93.8%vs.83.5%),the rate of 2 h postpartum bleeding(2.1%vs.7.2%)and the anxiety scores([54.51±4.21]vs.[63.49±3.98])(P<0.05).However,there was no statistical difference in the rate of neonatal asphyxia between the two groups(P>0.05).Conclusions The empowerment management program with authorization theory is an effective management measure during pregnancy,which can effectively improve the anxiety of first-time pregnant women,reduce the incidence of adverse risks during childbirth,improve the rate of natural delivery.It is worthy of clinical reference.
