摘　　要：目的分析降钙素原(PCT)、D-二聚体(D-D)、C反应蛋白(CRP)和淀粉酶(AMS)对重症肺炎患者的病情和预后的评估价值。方法取我院符合研究条件的病人76例,根据APACHEⅡ得分情况,分为3组,即A组25例(低于20分)、B组28例(20-30分)、C组23例(>30分)。根据恢复情况设置生存、死亡组,分别为49例和27例。检测各组PCT、D-D、CRP和AMS水平,并进行比较,分析APACHEⅡ评分与以上各检测指标的相关性。结果入院第1天,A组、B组和C组的各指标水平无显著差异(P>0.05);入院第3天和第7天,各指标水平C组>B组>A组,差异均有统计学意义(P<0.05)。死亡组各指标高于生存组,差异有统计学意义(P<0.05)。APACHEⅡ评分和PCT、D-D、CRP、AMS指标水平呈正相关(r=0.696、0.702、0.781、0.638,P<0.05)。结论PCT、D-D、CRP和AMS是评价重症肺炎病情的重要指标,各指标的检测可为病情的评估及预后的改善提供有效指导。ObjectiveTo analyze the value of procalcitonin(PCT),D-dimer(D-D),C-reactive protein(CRP)and amylase(AMS)in the condition and prognosis of patients with severe pneumonia.Methods76 patients with the research condition were enrolled between May 2018 and April 2020 in this study.According to the APACHEⅡscores,patients were divided into three groups,group A:less than 20 points(n=25),group B:20-30 points(n=28)and group C:more than 30(n=23).The survival and death groups were set according to the recovery conditions,which were respectively 49 cases and 27 cases.PCT,D-D,CRP and AMS level were detected,and analysis the correlation between APACHEⅡscore and the above detection index.ResultsThere was no significant difference among every indicator in group A,group B and group C at the first day after admission(P>0.05).Group C was greater than group B and greater than group A among each indicator at the 3rd and 7th day after admission(P<0.05).Secondly,the indicators in the death group were higher than those in the survival group(P<0.05).APACHEⅡscore and PCT,D-D,CRP,AMS index levels were positively correlated(r=0.696,0.702,0.781,0.638,P<0.05).ConclusionThe PCT,D-D,CRP and AMS are important indicators for the evaluation of severe pneumonia.The detection of each indicator can provide effective guidance for the evaluation of the disease and the improvement of prognosis.
