摘 要:目的探讨汉族T2DM患者结合珠蛋白(Hp)基因型和基因频率的分布特征与肾脏损伤指标的相关性。方法选取2016年8月至2018年8月于中南大学湘雅三医院内分泌科和肾内科住院治疗的汉族T2DM患者438例,另选取100名健康志愿者为正常对照(NC)组。采用PCR技术检测Hp基因型。结果依据UACR将T2DM患者分为正常白蛋白尿组(Normo,UACR<30 mg/g)、微量白蛋白尿组(Micro,UACR 30~300 mg/g)和大量白蛋白尿组(Macro,UACR>300 mg/g);另依据eGFR将T2DM患者分为CKD1组[≥90 ml/(min·1.73 m^(2))]、CKD2组[60~90 ml/(min·1.73 m^(2))]、CKD3组[30~60 ml/(min·1.73 m^(2))]和CKD4~5组[<30 ml/(min·1.73 m^(2))]。各组间Hp基因型和Hp等位基因频率比较,差异均无统计学意义(P>0.05)。不同Hp基因型T2DM患者临床资料和生化指标比较,差异均无统计学意义(P>0.05)。结论汉族T2DM患者Hp2-2基因型和Hp2等位基因频率显著高于NC组;未发现Hp基因多态性与汉族T2DM患者肾脏损伤的相关性。Objective To explore the distribution characteristics of haptoglobin(Hp)genotype and allele frequency in patient with type 2 diabetes mellitus(T2 DM)in Han population,and the association between genetic polymorphism in haptoglobin and kidney injury indexes.patients in Han population admitted to the Third Xiangya Hospital of Central South University between August 2016 and August 2018 and 100 healthy volunteers were enrolled in this study.Polymerase chain reaction(PCR)technology was used to detect the genotype and allele frequency of Hp.were further divided into three subgroups according to their UACR:Normal UACR group(Normo,UACR<30 mg/g),Micro UACR group(Micro,UACR 30~300 mg/g)and Macro UACR group(Macro,UACR>300 mg/g).Then T2 DM patients were divided into four subgroups according to their eGFR:CKD1 group[≥90 ml/(min·1.73 m^(2))],CKD2 group[60~90 ml/(min·1.73 m^(2))],CKD3 group[30~60 ml/(min·1.73 m^(2))]and CKD4~5 group[<30 ml/(min·1.73 m^(2))].However,there were no significant differences among the UACR-based subgroups nor eGFR-based subgroups(P>0.05).There were no statistically significant differences in clinical or biochemical indexes among T2 DM patients with different Hp genotypes(P>0.05).Conclusion The Hp2-2 genotype and Hp2 allele frequency was significantly higher in T2 DM patients in Han population than in control group.But no correlation was found between Hp genetic polymorphism and kidney injury in patients with T2 DM.
