摘　　要：目的分析促性腺激素释放激素(GnRHa)联合左炔诺孕酮宫内节育系统环(通用名:曼月乐)对子宫腺肌病患者痛经程度改善及子宫内膜厚度变化的影响。方法70例子宫腺肌病患者,根据随机数字表法分为对照组和联合治疗组,每组35例。对照组给予GnRHa治疗,联合治疗组在对照组的基础上给予联合曼月乐环治疗。比较两组患者月经疼痛评分、子宫内膜厚度、治疗效果以及不良反应发生情况。结果治疗后,两组患者月经疼痛评分、子宫内膜厚度均有所改善,且联合治疗组月经疼痛评分(1.52±0.21)分低于对照组的(2.72±0.45)分,子宫内膜厚度(4.56±0.12)mm低于对照组的(5.62±0.46)mm,差异均具有统计学意义(P<0.05)。联合治疗组治疗总有效率94.29%高于对照组的74.29%,差异具有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论曼月乐环联合GnRHa对于子宫腺肌病的治疗效果确切,可有效改善患者的月经情况,降低子宫内膜厚度,且可缓解痛经程度,具有较高的安全性,值得推广。Objective To analyze the effect of gonadotropin-releasing hormone agonist combined with levonorgestrel intrauterine system(trade name:Mirena)on the improvement of dysmenorrhea and the change of endometrial thickness in patients with adenomyosis.Methods A total of 70 patients with adenomyosis were divided into control group and combined treatment group according to random numerical table,with 35 cases in each group.The control group was treated with GnRHa,and the combined treatment group was treated with Mirena on the basis of the control group.The menstrual pain score,endometrial thickness and treatment effect were compared between the two groups.Results After treatment,the menstrual pain score and endometrial thickness of the two groups were better than those before treatment;the menstrual pain score(1.52±0.21)points was lower than(2.72±0.45)points of the control group,and endometrial thickness(4.56±0.12)mm was lower than(5.62±0.46)mm of the control group.All the difference was statistically significant(P<0.05).The total effective rate 94.29%of the combined treatment group was higher than 74.29%of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in incidence of adverse reactions between the two groups(P>0.05).Conclusion Combination of Mirena and GnRHa shows affirmative therapeutic effect on patients with adenomyosis,which can effectively improve the menstrual condition of patients,reduce endometrium thickness,and relieve the degree of dysmenorrhea.It has high safety and is worthy of promotion.
