摘　　要：目的探讨超低位去大骨瓣术联合亚低温对重症颅脑损伤患者脉搏指数(PI)与血乳酸(Jlac)水平的影响。方法回顾性分析2018年2月至2019年2月本院收治的重症颅脑损伤患者65例的临床资料,按照手术方法不同分为对照组(n=32)和观察组(n=33)。对照组采用超低位去大骨瓣术,观察组在对照组基础上采用亚低温治疗。比较两组术后6个月的临床疗效、PI指数及Jlac水平。结果术后6个月,观察组临床疗效优于对照组,术后5、10 d,观察组PI指数、Jlac水平均低于对照组,差异有统计学意义(P<0.05)。结论重症颅脑损伤患者采用超低位去大骨瓣术联合亚低温治疗疗效显著,可改善PI指数与Jlac水平,改善患者预后,值得临床推广应用。Objective To investigate the effect of ultra-low position craniectomy combined with mild hypothermia on pulse index(PI)and blood lactic acid(Jlac)levels in patients with severe craniocerebral injury.Methods The clinical data of 65 patients with severe craniocerebral injury from February 2018 to February 2019 were analyzed retrospectively,they were divided into control group(n=32)and observation group(n=33).The control group was treated with ultralow osteotomy,the observation group was treated with mild hypothermia on the basis of the control group.The clinical efficacy,PI index and Jlac level were compared between the two groups at 6 months after operation.Results 6 months after surgery,the clinical effect of the observation group was better than that of the control group,at 5 and 10 days after operation,the PI index and Jlac level of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).Conclusion The treatment of severe craniocerebral injury with ultra-low-position craniectomy combined with mild hypothermia is effective,can improve PI index,Jlac levels and the prognosis,it is worthy of clinical application.
