摘 要:目的探讨零切迹颈前路融合器在单节段脊髓型颈椎病患者手术中的应用。方法选取2018年3月至2020年3月本院收治的60例单节段脊髓型颈椎病患者作为研究对象,按随机对照原则分为两组,各30例。对照组给予传统钉板融合器内固定术治疗,观察组给予零切迹颈前路融合器治疗。比较两组不同时点日本骨科协会评估治疗(JOA)评分、侧弯角度大小(Cobb角)及并发症发生率。结果两组术后1、3个月评分均高于术前,差异有统计学意义(P<0.05);观察组术后1、3个月JOA评分均高于对照组,但差异无统计学意义。两组术后1、3个月评分均高于术前,差异有统计学意义(P<0.05);观察组术后1、3个月Cobb角均高于对照组,但差异无统计学意义。观察组并发症率(3.33%)低于对照组(13.33%),但差异无统计学意义。结论零切迹颈前路融合器能有效改善单节段脊髓型颈椎病患者机体功能及活动度,利于减少术中损伤,降低并发症发生率,值得临床推广应用。Objective To explore the application of zero-notch anterior cervical fusion cage in the operation of patients with single-segment cervical spondylotic myelopathy.Methods 60 patients with single-segment cervical spondylotic myelopathy who were treated in our hospital from March 2018 to March 2020 were selected as the research subjects,and they were divided into two groups according to the principle of randomized control,with 30 cases in each group.The control group was treated with traditional nail plate fusion cage internal fixation,and the observation group was treated with zero-notch anterior cervical fusion cage.The Japan Orthopaedic Association assessment treatment(JOA)score,scoliosis angle(Cobb angle)and incidence of complications in the two groups were compared.Results The JOA scores of the two groups 1 month and 3 months after operation were higher than those before the operation,and the difference was statistically significant(P<0.05);the JOA score in the observation group was slightly higher than the control group 1 month and 3 months after operation,but the difference was not statistically significant;the Cobb angle 1 month and 3 months after operation in the two groups was higher than that before operation,the difference was statistically significant(P<0.05);the Cobb angle in the observation group was slightly higher than that in the control group 1 month and 3 months after operation,but the difference was not statistically significant;the complication rate of the observation group(3.33%)was lower than that of the control group(13.33%),the difference was not statistically significant.Conclusion Zero-notch anterior cervical fusion cage can effectively improve the body function and mobility of patients with single-segment cervical spondylotic myelopathy,which is beneficial to reduce intraoperative injuries and reduce the incidence of complications,which is worthy of clinical promotion and application.
