摘 要:目的探讨复合皮移植联合亲水性银离子辅料治疗足踝部深度烧伤的疗效评估及对血清白蛋白(ALB)、细胞间粘附分子-1(ICAM-1)、可溶性E-选择素的影响。方法选取2019年2月至2020年2月本院收治的110例足踝部深度烧伤患者作为研究对象,按照随机数字表法分为研究组与对照组,每组55例。对照组予以复合皮移植及常规换药治疗,观察组在对照组基础上予以亲水性银离子辅料治疗,比较两组创面愈合情况(创面愈合时间、换药次数、创面愈合率)、血清指标(ALB、ICAM-1、可溶性E-选择素)及创面分泌物细菌检出率。结果研究组创面愈合时间短于对照组,换药次数少于对照组,差异具有统计学意义(P<0.05)。治疗后1、2、3周,研究组创面愈合率均高于对照组,差异具有统计学意义(P<0.05)。治疗后,两组血清ICAM-1、可溶性E-选择素均低于治疗前,且观察组均低于对照组(P<0.05);治疗后,两组血清ALB均高于治疗前,且研究组高于对照组(P<0.05)。治疗1周后,对照组创面分泌物检测细菌阳性2例(3.64%),而研究组仅出现1例(1.82%),两组比较差异无统计学意义(χ^(2)=0.346,P=0.556);用药2周后,两组创面均未检测出细菌。结论复合皮移植联合亲水性银离子辅料治疗足踝部深度烧伤效果明显,可明显降低血清细胞间粘附分子-1、可溶性E-选择素水平,提高血清白蛋白,值得临床推广应用。Objective To explore the effect of composite skin grafting combined with hydrophilic silver ion adjuvant in the treatment of deep burns of the foot and ankle and the effect on serum ALB,ICAM-1 and soluble E-selectin.Methods 110 patients with deep burns of foot and ankle who were admitted to our hospital from February 2019 to February 2020 were selected as the research subjects.They were divided into study group and control group according to random number table method,with 55 cases in each group.The control group was given compound skin grafting and routine dressing change,and the observation group was given hydrophilic silver ion adjuvant treatment on the basis of the control group.The wound healing conditions(wound healing time,dressing change times,wound healing rate),serum indicators(ALB,ICAM-1,soluble E-selectin)and detection rate of wound secretion bacteria between the two groups were compared.Results The wound healing time of the study group was shorter than that of the control group,and the number of dressing changes was less than that of the control group,the difference was statistically significant(P<0.05).At 1,2,and 3 weeks after treatment,the wound healing rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the serum ICAM-1 and soluble E-selectin of the two groups were lower than before treatment,and the observation group was lower than the control group(P<0.05);after treatment,the serum ALB of the two groups were higher than before treatment,and the study group was higher than the control group(P<0.05).After 1 week of treatment,2 cases(3.64%)of wound secretions in the control group were positive for bacteria,but only 1 case(1.82%)in the study group.There was no statistically significant difference between the two groups(χ^(2)=0.346,P=0.556);after 2 weeks of medication,no bacteria were detected on the wounds of the two groups.Conclusion Compound skin grafting combined with hydrophilic silver ion excipient in the tr
