脾胃病治疗中的“调中复衡”理论

摘　　要：脾胃学说、脾虚理论、补土理论、通降论被广泛应用于脾胃病临床,在继承传统脾胃理论及通降论学术思想基础上,结合长期临床经验,提出了治疗脾胃病的"调中复衡"理论,倡导脾胃升降相因,降在先;纳化相协,调为要;脾胃辨治,论分合;五脏调理,土为枢。在临床应用方面提出了"治脾八法""八纲通胃法""两平衡"策略。治脾八法即健脾理气、健脾化湿、健脾温阳、健脾渗湿、温脾止泻、健脾柔肝、健脾宣肺、温补脾肾法。八纲通胃法即从脏腑、虚实、气血、寒热八纲论治胃病。两平衡即恢复气机升降平衡、恢复脏腑功能平衡。The theory of spleen and stomach, theory of spleen deficiency, theory of replenishing earth, and theory of unblocking and descending are put forward and have been widely used in treating spleen and stomach diseases. Based on the above mentioned theories and long-term clinical experience, the theory of "harmonizing the center to restore balance" is proposed, which advocates that ascending and descending methods should be used together to regulate spleen and stomach, and descending method should be set as priority;intaking and transforming methods are combined when regulation is the key;syndrome differentiation of spleen and stomach is adopted to determine a separated or combined treatment;the earth(spleen) is the pivot when regulating five zang organs(五脏). In terms of clinical application, the strategies including "eight spleen-fortifying methods", "eight principles of unblocking stomach" and "two balances" are put forward. The "eight splee-fortifying methods" refer to fortifying spleen through rectifying qi, removing dampness, warming yang, percolating dampness, arresting diarrhea, softening the liver, diffusing the lung, as well as warming and supplementing spleen and kidney. The "eight principles of unblocking stomach" refer to treating stomach diseases with zang-fu(脏腑), deficiency-excess, qi-blood, cold-heat pattern differentiation. The "two balances" are the re-balance of ascending and descending of qi movement and function of zang-fu.
