摘  要:目的研究寻常型银屑病不同证候唾液差异代谢物,探讨其证候物质基础。方法招募寻常型银屑病血热证(21例)、血瘀证(13例)、血燥证(11例)患者为疾病组,以及同时期健康志愿者30例设为正常对照组。采用UPLC-MS/MS检测平台、自建数据库以及多元统计分析相结合的方法研究寻常型银屑病各证型组与正常对照组受试者唾液样本代谢组差异,经主成分分析筛选证候相关的生物标志物,利用KEGG数据库对筛选的差异代谢物进行注释和通路富集。结果 75份唾液样本共检测到368个代谢物,与正常对照组比较,特征性差异代谢物血热证组有3-(3-羟基苯基)丙酸等9个,血瘀证组有环己基氨基磺酸等7个,血燥证组有月桂酸等4个。3个证型组组间两两比较,血热证组相比血瘀证组检测出10个差异代谢物,血热证组相比血燥证组检测出5个差异代谢物,血瘀证组相比血燥证组检测出17个差异代谢物。差异代谢物主要涉及脂类、有机酸、氨基酸、核苷酸及其衍生物等代谢类别,显著富集的代谢通路有甘油磷脂代谢通路等5条。结论寻常型银屑病患者血热证、血瘀证、血燥证唾液代谢水平均存在显著差异,溶血卵磷脂酰胆碱、2-氨基己二酸、皮质醇等多个差异代谢物可能作为证候分型潜在的生物标志物,特异性代谢通路涉及甘油磷脂代谢通路等。Objective To explore the material basis of different patterns of psoriasis vulgaris(PV) by analyzing the salivary metabolic profiles. Methods Forty-five PV patients with blood heat pattern(21 cases), blood stasis pattern(13 cases), or blood dryness pattern(11 cases) were recruited as the disease group, while 30 healthy volunteers during the same period were recruited as the normal control group. The UPLC-MS/MS detection platform, self-built database, and multivariate statistical analysis were used to compare the differences in the metabolites of saliva samples between different PV pattern groups and the normal control group;principal component analysis was used to screen syndrome-related biomarkers, and the KEGG database was used to annotate and enrich the pathways of the selected differential metabolites. Results A total of 368 metabolites were detected in 75 saliva samples. When compared with the metabolites of the normal control group, there were nine specific differential metabolites such as 3-(3-hydroxyphenyl) propionic acid identified in blood heat pattern patients, seven differential metabolites such as cyclohexyl-sulfamic acid identified in blood stasis pattern patients, and four differential metabolites such as lauric acid identified in blood dryness pattern patients. Pairwise comparison analysis between blood heat pattern and blood stasis pattern group identified 10 differential metabolites;comparison between blood heat pattern and blood dryness pattern group found 5 differential metabolites;comparison between blood stasis pattern and blood dryness pattern group found 17 differential metabolites. The categories of the identified differential metabolites mainly include lipids, organic acids, amino acids, nucleotides and their derivatives. There were five significantly enriched metabolic pathways such as glycerophospholipid pathway.
