摘　　要：目的:探讨指动脉逆行岛状皮瓣、筋膜皮支皮瓣的临床应用及疗效。方法:2013年10月-2019年9月于福建中医药大学附属泉州市正骨医院手外科应用指动脉逆行岛状皮瓣、指动脉筋膜皮支皮瓣修复手指皮肤缺损(简称手指皮肤缺损)患者49例,随机分为皮支组24例、逆行组25例两组并进行比较,对两组病例皮瓣的大小、手术时间、出血量、术后并发症、住院长短、皮瓣的外观、感觉功能、手指活动功能进行比较。结果:两组术后没有皮瓣发生坏死,患指活动恢复良好,两组在修复的创面大小、外观、住院时间方面差异无统计学意义,术后血运逆行组回流问题较多、皮支组供血问题较多,而皮支组创伤少、手术方法简单、手术时间短、感觉恢复优于逆行组。结论:指动脉逆行岛状皮瓣术后供血较好,但静脉回流较差,筋膜皮支皮瓣供血较差,但筋膜皮支皮瓣在创伤、手术时间、术中出血量、感觉恢复等方面更有优势。Objective:To explore the clinical application and effect of digital artery retrograde island flap and fascial skin flap.Methods:From October 2013 to September 2019,49 patients with finger skin defect were treated with digital artery retrograde island flap and finger artery fascia skin branch flap in hand surgery of Quanzhou Orthopedic-traumatological Hospital.The patients were randomly divided into 24 cases of fascial skin flap group and 25 cases of retrograde group.The size,operation time,bleeding volume,postoperative complications,and length of hospitalization,appearance,sensory function and finger movement function of the flap were compared between the two groups.Results:There was no necrosis of the flap in the two groups after operation,and the activity of the affected finger recovered well,and there was no significant difference in the size,appearance and hospitalization time of the repaired wound between the two groups.There were more reflux problems in the retrograde group and more blood supply problems in the fascial skin flap group,but fascial skin flap group had less trauma,simple operation method,short operation time and better sensory recovery.Conclusion:The blood supply of the finger artery retrograde island flap is better after operation,but the venous reflux is poor,and the blood supply of the fascial skin branch flap is poor,but it has more advantages in trauma,operation time,intraoperative blood loss,sensory recovery and so on.
