穴位刺激联合硬膏贴敷治疗双J管置入术后相关并发症的疗效

摘　　要：目的:观察经皮穴位刺激联合硬膏贴敷对治疗双J管置入术后相关并发症的临床疗效。方法:选取2017年5月-2019年2月在驻马店市中医院住院并行碎石手术治疗(经皮肾镜碎石、输尿管镜碎石、输尿管软镜碎石)的上尿路结石患者194例,按照随机化原则将患者分为两组。对照组术后给予常规处理,观察组在常规处理基础上给穴位刺激联合中药硬膏贴敷。比较两组患者输尿管支架管留置期间视觉模拟评分法(Visual Analogue Scale,VAS)疼痛评分、血尿、膀胱刺激征、生活质量(Quality of Life,QOL)评分情况。结果:194例患者均在术后1个月拔除输尿管支架管,并完成术后的随访及相关的问卷。术后24 h两组患者的腰痛症状评分、肉眼血尿症状、膀胱刺激征、VAS、QOL评分差异无统计学意义(P>0.05);观察组术后5 d、拔除双J管当天的肉眼血尿症状、膀胱刺激征改善明显优于对照组,视觉疼痛模拟评分低于对照组,生活质量优于对照组,差异具有统计学意义(P<0.05)。结论:穴位刺激联合硬膏贴敷能有效改善碎石术后留置双J管引起的并发症,提高患者生活质量,值得临床推广应用。Objective:To evaluate the clinical effect of transcutaneous acupoint stimulation and acupoint application in the treatment of double J tube related symptom.Method:From May 2017 to February 2019,194 upper urinary stones patients who were treated by percutaneous nephrolithotomy,ureteroscopic lithotripsy,and flexible ureteroscopic lithotripsy in Zhumadian Hospital of Traditional Chinese Medicine were randomly divided into two groups.The control group was given routine treatment after operation.The observation group was given acupoint electric stimulation and acupoint application on the basis of routine treatment after the first day of the operation.The score of visual analogue scale(VAS),hematuria,bladder irritation and quality of life score(QOL)were compared between the two groups.Results:All patients in this study underwent removal of ureteral stent 1 month after operation and completed follow-up and related questionnaires.There was no significant difference in lumbago symptom score,hematuria,bladder irritation,VAS and QOL between the two groups 24 hours after operation(P>0.05).5 days after operation and on the day of double J tube removal,the hematuria and bladder irritation symptoms in the observation group were more significantly relieved,VAS score of the observation group was lower than that of the control group,the quality of life of the observation group was better than that of the control group(P<0.05).Conclusion:Acupoint stimulation and acupoint application can reduce double J tube-related symptom and improve the quality of life,which is worth clinical application.
