摘　　要：介绍郭长青教授运用针刀治疗腰椎间盘突出症的临床经验。郭教授认为本病需标本兼治,方能取得速效而不易复发;提出针刀治疗点需综合参照症状特点、查体及影像表现等进行选择,贵精而不贵多;注重神经定位与经络辨证相结合;认为在神经通路上的多处松解可有效降低神经总体的纵向张力,改善卡压处微循环,缓解水肿;在治疗上主张上下共取、腰腿同治,精确选择腰椎及下肢的卡压点后一并松解,整体解除受累神经的异常张力,促进神经功能恢复;认为针刀起效机制除了减张减压的机械作用外还能改善局部血液循环,促进炎性物质排出及神经脱敏;注重对腰椎整体稳定性的恢复,调节周围软组织张力,解除关节功能障碍,恢复脊柱生物力学平衡,避免髓核突出再次加重,从力学层面改善症状并降低复发率;擅长结合中药内服,辅助增强外治疗效,并提出分期辨证用药的思路。郭长青教授运用针刀治疗腰椎间盘突出症取得了良好的临床疗效。Professor Guo Changqing's clinical experience in the treatment of lumbar disc herniation with acupotomy is introduced.Professor Guo thinks that the disease needs treatment of both symptoms and the root causes,so as to achieve quick effect and it is not easy to recur.He puts forward that acupotomy treatment points should be selected by referring to symptom characteristics,physical examination and image performance comprehensively.It's quality rather than quantity that counts.He pays attention to the combination of nerve location and meridian syndrome differentiation.He thinks that multiple release on nerve pathway can effectively reduce the overall longitudinal tension of the nerve,improve microcirculation at the entrapment site,and relieve the entrapment edema;it is suggested that the upper and lower parts of the treatment should be taken together,the waist and legs should be treated together,the compression points of the lumbar spine and lower limbs should be accurately selected and then released together,so as to relieve the abnormal tension of the involved nerves as a whole and promote the recovery of nerve function;it is believed that the mechanism of acupotomy can not only reduce the mechanical effect of tension and decompression,but also improve the local blood circulation,promote the discharge of inflammatory substances and desensitization of nerves;pay attention to the overall stability of the lumbar spine.In order to recover,it is important to adjust the tension of surrounding soft tissue,relieve the joint dysfunction,restore the biomechanical balance of spine,avoid the aggravation of nucleus pulposus again,improve the symptoms and reduce the recurrence rate from the mechanical level;Professor Guo is good at combining the internal administration of traditional Chinese medicine,assisting to enhance the external treatment effect,and he puts forward the idea of drug differentiation by stages,which has achieved good clinical effect.
