李坤寅基于“带瘤妊娠”论治子宫肌瘤生育障碍

摘　　要：子宫肌瘤是女性常见的良性肿瘤之一,常伴随生育障碍。对于高龄有生育要求的子宫肌瘤患者,李坤寅教授认为可以考虑带瘤妊娠。子宫肌瘤生育障碍患者病机虚实夹杂,表现为肾脾两虚兼见肝郁。基于"带瘤妊娠"理念,通过活血化瘀、祛痰、消食导滞之法以消散癥块,补肾健脾之法以培元扶正,达到消补兼施、分期论治;治病与安胎并举;重视患者情志、饮食调摄等多途径治疗。"带瘤妊娠"理念为整体观与辨证论治的有机结合,可为患者实现带瘤妊娠提供思路。Hysteromyoma is one of the common benign tumors in women, usually accompanying with fertility disorders. For the elderly hysteromyoma women who have the child-bearing desire, Professor LI Kun-Yin advocates "tumor-bearing pregnancy". The pathogenesis of hysteromyoma patients with fertility disorder is characterized by the mixture of deficiency and excess,and is usually manifested as deficiency of both kidney and spleen complicated with liver stagnation. Following the concept of "tumor-bearing pregnancy",Professor LI Kun-Yin adopts the methods of activating blood to resolve stasis,removing phlegm,and promoting digestion to remove food retention for dissipating abdominal mass during the treatment of hysteromyoma patients with fertility disorder.Meanwhile, Professor LI Kun-Yin adopts the therapy of tonifying kidney and invigorating spleen to replenish primordial qi and support healthy qi, so as to achieve the aim of simultaneous utilization of tonification and resolution and staging treatment. During the treatment of hysteromyoma, preventing miscarriage should be noticed,and the emotional and dietary regulation is also stressed. The thought of "tumor-bearing pregnancy"is the combination of holistic concept with syndrome differentiation and treatment,and will provide thoughts for the successful tumor-bearing pregnancy.
