儿童急性荨麻疹发病与隐匿性链球菌感染的相关性

摘　　要：目的:拟证实隐匿性链球菌感染与儿童急性荨麻疹(AU)发病相关。方法:于2015至2018年招募湖北省中医院29例3~6岁和27例7~14岁儿童特发性荨麻疹患者(AU组)和两个年龄段各15例健康对照者(对照组)。检测血常规、血清超敏C反应蛋白(hsCRP)及血清链球菌抗体(ASP、ASO)水平。结果:白细胞计数、hsCRP异常升高的占比在3~6岁组患者为69.0%和82.8%,在7~14岁组患者为63.0%和85.1%。两组患者白细胞和中性粒细胞数都显著高于健康对照者(P<0.01)。ASP和ASO抗体阳性率在3~6岁组患者为65.5%和58.6%,在7~14岁组患者为50.3%和66.7%。两组均有很大比例(30%~60%)患者表现出白细胞升高同时伴有ASP或/和ASO抗体阳性。结论:隐匿性链球菌感染很可能是儿童急性荨麻疹的诱因。Objective: To evaluate whether streptococcal infection is a cause for acute urticaria(AU) in children.Methods: Twenty-nine children from 3 to 6 years old and 27 children from 7 to 14 years old with spontaneous AU attack were recruited, as well as 30 health children were included as controls.Complete blood counts, hsCRP levels, antibody levels of streptococcal(ASP and ASO) were tested.Results: The percentages of AU patients with abnormally elevated white blood cell count(WBC) and hsCRP level were 69. 0% and 82. 8% respectively in 3-6 years old group, 63. 0% and 85. 1% respectively in 7-14 years old group. Leukocyte and neutrophil counts in patients from both groups were significantly higher than those of health controls(P<0. 01). Positive rates of ASP and ASO antibodies were 65. 5% and 58. 6% respectively in 3-6 years old group, as well as 50. 3% and 66. 7% respectively in 7-14 years old group. Furthermore, elevated WBC accompanied with positive ASP or/and ASO antibodies were observed in AU patients in both groups.Conclusion: Occult streptococcal infection might be a trigger for AU in children.
