摘 要:目的 分析本院2013年-2019年A群链球菌的耐药特征及流行病学特征。方法 回顾性分析2013年-2019年本院细菌培养为A群链球菌患儿的流行病学特点。结果 分离到GAS的患儿3 612例。74. 3%的菌株来自4岁~7岁患儿;来自咽拭子标本占98. 1%,37. 7%的患儿首诊为猩红热,无侵袭性感染;4月-6月检出菌株占43. 6%。进行药敏检测的3 239株GAS对青霉素、氨苄西林、头孢噻肟、头孢曲松、头孢吡肟、万古霉素全部敏感,对左氧氟沙星、红霉素、克林霉素耐药率分别为0. 1%、95. 4%、95. 3%。复诊者中,67. 9%用药5 d内培养转阴。结论 本地区A群链球菌对红霉素和克林霉素耐药率极高,对青霉素保持敏感,临床仍可选用β-内酰胺类药物为治疗该菌感染的首选。Objective To investigate antibiotie resistance and epidemiological characteristics of Group A Streptococeus isolated from children in 2013-2019.Methods A retrospective analysis on epidemiological characteristics of children with group A Streptococcus bacteria culture in our hospital from 2013 to 2019.Results Group A Streptococcus was isolated from 3612 patients,of which 74.3%strains were from children aged 4-7 years and 98.1%strains were from pharynx swabs,37.7%children were diagnosed as scarlet fever for the visit to doctors,and there was no invasive infection in 7 years.The isolates from April to June accounted for 43.6%.All 3239 strains with drug susceptibility data were sensitive to pernieilin,ampieillin,cefotaxime,cefriaxone,cefepime and vacomyein.The antibiotic resistance rates to levofloxacin,erythrocin,clindamyein was 0.1%,95.4%,95.3%respectively.67.9%of the patients returned to the clinic turmned negative within 5 d after treatment.Conclusion Group A Streptococcus was extremely high in antibiotic resistance rate for erythrocin and clindamycin,but was still sensitive to penicillin;therefore pernicilin and other beta-lactam antibiotics are still the first choice to treat infections caused by Group A Streptococcus.
