摘　　要：目的 探讨外周血高荧光强度淋巴细胞(HFLC)在EB病毒感染相关疾病中的变化及应用价值。方法 选取2018年11月-2019年10月本院门诊和住院患者,临床症状主要表现为发热、咽峡炎、扁桃体炎、淋巴结肿大、皮疹以及肝脾肿大等EB病毒感染高危人群作为实验组190例,选取同期健康体检者作为对照组60例,RT-PCR检测其外周血EBV-DNA拷贝数确定是否为EBV感染,将实验组分为EBV阳性组(89例)和EBV阴性组(101例),通过Sysmex XN-9000血细胞分析仪定量检测外周血高荧光强度淋巴细胞参数(HFLC百分比和HFLC绝对值),人工镜检异型淋巴细胞(ALY),比较HFLC百分比、HFLC绝对值在EBV阳性组和EBV阴性组的变化程度,分析HFLC与异型淋巴细胞百分比、EBV-DNA拷贝数之间的相关性。结果 EBV感染阳性组HFLC百分比、HFLC绝对值、ALY百分比、EBV-DNA拷贝数均高于EBV感染阴性组及对照组(P <0. 05);EBV感染阴性组HFLC百分比、HFLC绝对值、ALY百分比均高于对照组(P <0. 01)。HFLC与ALY百分比、EBV-DNA拷贝数呈正相关。HFLC百分比、HFLC绝对值、ALY百分比诊断EB病毒感染相关性疾病的曲线下面积分别为0. 750、0. 737及0. 782,当HFLC百分比的界值为1. 75%,敏感度为69. 7%,特异度为69. 9%;当HFLC绝对值的界值为0. 075×10^(9)/L,敏感度为78. 7%,特异度为55. 3%;当ALY百分比的界值为6. 50%,敏感度为59. 6%,特异度为95. 0%。结论 外周血HFLC可作EB病毒感染相关疾病筛查一种快速、经济、简便的定量检测新指标,指导临床有针对性的进一步检查和辅助治疗提供实验依据。Objective To investigate the changes and application value of peripheral blood high fluorescence intensity lympho-cytes(HFLC)in Epstein-Barr virus infection-related diseases.Methods A total of 190 cases in our hospital outpatient and inpatient from November 2018 to October 2019 were selected as experimental group,who were under high-risk of EB virus infeetion with the main clinical symptoms of fever,angina,tonsillitis,swollen lymph nodes,rash and hepatosplenomegaly,and 60 cases for healthy physical examination in the same period were selected as the control group.The peripheral blood EBV-DNA copy number was detected by RT-PCR to determine whether the patients were infected with EBV,and the experimental groups were divided into the EBV positive group(89 cases)and EBV negative group(101 cases),peripheral blood lymphocyte parameters of high fluorescence intensity(HFLC%and HFLC#)were quantitatively detected by Sysmex XN-9000 hematocyte analyzer,and atypical lymphocytes were examined under microscope.The change degree of HFLC%and HFLC#in EBV posi-tive group and EBV negative group was compared,and the correlation between HFLC and percentage of atypical lymphoeytes and EBV-DNA copy number was analyzed.Results The HFLC%,HFLC#,atypical lymphocyte and EB-DNA copy value in EBV infection positive group were higher than those in EBV infection negative group and the control group(P<0.05).HFLC%,HFLC#,and atypical lymphocytes in EBV negative group were all higher than that in healthy control group(P<0.01).HFLC in peripheral blood were positively correlated with atypical lymphocytes and EBV-DNA copy value.The area under ROC curve of HFLC%,HFLC#,and atypical lymphocytes were 0.750,0.737 and 0.782,respectively,for the diagnosis of Epstein-Barr virus infection-related diseases.When the threshold of HFLC%was 1.75%,the sensitivity was 69.7%and the speificity was 69.9%。When the threshold value of HFLC#was 0.075 × 10^(9)/L,the sensitivity was 78.7%and the speei-ficity was 55.3%.When the threshold value of atypical lymphocytes was 6.5
