摘　　要：目的分析梅毒血清固定发病人群Treg细胞、可溶性细胞毒性T淋巴细胞相关蛋白4(solubility cytotoxic T-lymphocyte-associated protein 4,sCTLA-4)和梅毒螺旋体IgM抗体(Treponema pallidum IgM,TP-IgM)表达情况与梅毒血清固定的关系。方法以2017年1月到2019年12月诊断的梅毒血清固定患者80例为研究对象,另选取健康患者80例为对照组。分析两组患者以及不同疾病组患者的Treg细胞、sCTLA-4和TP-IgM表达情况之间的差异。分析患者的Treg细胞、sCTLA-4和TP-IgM表达情况与快速血浆反应素环状卡片试验(Resilient Packet Ring,RPR)滴度水平的相关性。结果疾病组患者的Treg细胞显著低于健康组(t=19.754,P<0.01),sCTLA-4(t=42.901,P<0.01)和TP-IgM(χ^(2)=137.241,P<0.01)表达情况显著高于对照组,不同患者的Treg细胞(F=12.236,P<0.01)、sCTLA-4(F=14.265,P<0.01)和TP-IgM(χ^(2)=11.036,P<0.01)表达情况之间的差异有统计学意义。随着患者的RPR滴度水平的升高,患者的Treg细胞水平显著降低,sCTLA-4和TP-IgM表达显著升高,患者的RPR水平与患者的Treg细胞水平呈现负相关,与sCTLA-4和TP-IgM表达水平呈现正相关。结论梅毒血清固定发病与Treg细胞、sCTLA-4和TP-IgM表达情况显著相关,可作为日后梅毒血清固定发病的重要预测指标。Objective To analyze the relations between Treg cells,solubility cytotoxic T-lymphocyte-associated protein 4(sCTLA-4),Treponema pallidum IgM(TP-IgM)expression and syphilis serofixation.Methods Eighty syphilis patients diagnosed from January 2017 to December 2019 were selected as study subjects,and another 80 healthy patients were divided into the control group.The differences in expressions of Treg cells,sCTLA-4 and TP-IgM of patients in two groups and patients with different diseases in groups were analyzed.The correlation between Treg cells,sCTLA-4 and TP-IgM expression in patients and the titer level of rapid plasma Resilient Packet Ring(RPR)was analyzed.Results Treg cells of patients in the disease group were significantly lower than those of patients in the healthy group(t=19.754,P<0.01),and the expressions of sCTLA-4(t=42.901,P<0.01)and TP-IgM(χ^(2)=137.241,P<0.01)in the disease group were significantly higher than such indexes in the control group.The differences in the expressions of Treg cells(F=12.236,P<0.01),sCTLA-4(F=14.265,P<0.01)and TP-IgM(χ^(2)=11.036,P<0.01)in different patients were statistically significant,With the increase of RPR titer,the level of Treg cells decreased significantly,and the expression of sCTLA-4 and TP-IgM increased significantly,and the level of RPR was negatively correlated with the level of Treg cells and positively correlated with the expression of sCTLA-4 and TP-IgM.Conclusion The syphilis serofixation is significantly related to the expressions of Treg cells,sCTLA-4 and TP-IgM,which are eligible to be used as important predictors of syphilis serofixation in the future.
