摘  要:目的评价孤立性轻中度对称/不对称性侧脑室扩张(脑室宽度10~15 mm)胎儿不同脑区表观扩散系数(apparent diffusion coefficient,ADC)值的变化。方法对本院2015年3月至2019年6月期间经核磁共振扫描确诊的54例孤立性轻中度侧脑室扩张的胎儿与40例正常胎儿(对照组)进行比较;54例胎儿被分成:非对称性脑室扩张组(双侧脑室宽度为10~15 mm,双侧宽度差≥2 mm,AVM组)6例,对称性脑室扩张组(双侧脑室宽度为10~15 mm,双侧宽度差<2 mm,SVM组)10例和单侧脑室扩张组(一侧脑室宽度为10~15 mm,另一侧<10 mm,AV1norm组)38例;分别测量不同脑区的ADC值。结果基底节区:对照组ADC值为(1.28±0.15)×10^(-6)mm^(2)/s,SVM组为(1.39±0.13)×10^(-6)mm^(2)/s,AV1norm组扩张侧为(1.35±0.16)×10^(-6)mm^(2)/s,SVM组和AV1norm组的扩张侧基底节区ADC值均较正常对照组显著升高(均P<0.05)。AV1norm组中脑室扩张侧基底节ADC值较脑室正常侧显著升高(P<0.05),但是对照组、AVM组和SVM组双侧基底节区ADC值差异均无统计学意义。额叶:对照组ADC值为(1.53±0.17)×10^(-6)mm^(2)/s,SVM组为(1.39±0.18)×10^(-6)mm^(2)/s,AVM组为(1.36±0.17)×10^(-6)mm^(2)/s,AVM组和SVM组额叶ADC值均较对照组显著降低(均P<0.05)。结论不同类型孤立性轻中度侧脑室扩张胎儿的不同脑区ADC值有其特异性变化,有助于研究不同类型孤立性轻中度侧脑室扩张的病理生理学变化。Objectives To evaluate the impact of symmetric and asymmetric isolated mild and moderate ventriculomegaly(ventricle width 10-15 mm)on apparent diffusion coefficient(ADC)values in fetal brain areas.Methods Fifty-four fetuses with isolated mild and moderate ventriculomegaly diagnosed by sequential fetal head magnetic resonance imaging scan at our hospital from March,2015 to June,2019 were compared with 40 normal fetuses,a control group.The 54 cases were divided into an asymmetrical ventriculomegaly group(n=6),whose both ventricle widths were 10-15 mm,with a≥2 mm difference,a symmetrical ventriculomegaly group(n=10),whose both ventricle widths were 10-15 mm,with a<2 mm difference,and a ventriculomegaly on one side group(n=38),whose one ventricle width was 10-15 mm and the other one<10 mm.Their brain areas'ADC values were measured.Results In the basal ganglia,the ADC value was(1.28±0.15)×10^(-6) mm^(2)/s in the control group,was(1.39±0.13)×10^(-6) mm^(2)/s in the symmetrical ventriculomegaly group,and was(1.35±0.16)×10^(-6) mm^(2)/s on the ventriculomegaly side of the ventriculomegaly on one side group;the ADC values in the symmetrical ventriculomegaly group and in the basal ganglia on the ventriculomegaly side of the ventriculomegaly on one side group were higher than that in the control group(both P<0.05).In the ventriculomegaly on one side group,the ADC value in the basal ganglia was higher on the ventriculomegaly side than on the other side(P<0.05).In the frontal lobes,the ADC value was(1.53±0.17)×10^(-6) mm^(2)/s in the control group,was(1.39±0.18)×10-6mm2/s in the symmetrical ventriculomegaly group,and was(1.36±0.28)×10-6mm2/s in the asymmetrical ventriculomegaly group;the ADC values in the frontal lobes of the symmetrical ventriculomegaly group and the asymmetrical ventriculomegaly group significantly decreased(both P<0.05).Conclusion Fetal isolated mild and moderate ventriculomegaly of different types is associated with distinct ADC values in different brain areas,so it is valuable to study the
