摘　　要：本文通过对1例交通事故方向盘伤继发布加综合征案例进行研究,探讨外伤性布加综合征的认定标准以及伤残评定时需要考虑的因素。本例方向盘伤具有相当严重的暴力程度,影像学检查证实下腔静脉血管损伤,其伤后存在可致血液高凝及血流动力学改变的诸多因素如应激状态、肝功能损害、止血治疗、卧床休养、血管壁损伤,同时可以排除其他自身疾病的情形,因此认定本例为外伤性布加综合征。总结本文案例后,笔者认为认定外伤性布加综合征要依据案情、致伤方式、病史、辅助检查、治疗手段等综合分析。Through exploration into a traffic accident case that Budd-Chiari syndrome was suspicious of secondary to the injuries from steering wheel,the approbatory criterion was thereby discussed about evaluating traumatic Budd-Chiari syndrome,having its relevant appraisal of disability grade deliberated on the causation necessary to consider.The impacting steering wheel resulted in serious trauma to the injured woman in the case,leaving her at suffering from vascular injuries in inferior vena cava observed under imaging examination.The various post-injury existing factors caused her blood hypercoagulation,hemodynamic changes and abnormal living situations,having led her into the status of,e.g.,stress state,liver function damage,hemostasis treatment,bedding rest and vascular wall injury,any of such complications against which her self-diseases,if likely,would not have developed.Hence,the symptom of the injured person in this case was diagnosed as traumatic Budd-Chiari syndrome.The scrutiny of this case inspires an insight that the identification of traumatic Budd-Chiari syndrome should be conducted on the comprehensive analysis into the involving case,injury manner,medical record,auxiliary examination and treatment.
