摘  要:目的:探讨320排容积CT应用低管电压结合管电流自动调制技术、固定噪声指数(SD)进行门静脉成像,评价其对图像质量影响及降低辐射剂量可行性。方法:连续选取119例腹部CT检查者随机分A、B两组。采用管电流调制扫描技术,分别采用100KV(A组)、120KV (B组)行腹部CT检查,2组预设SD均为7.5,记录2组患者门静脉期毫安秒(mAs)、扫描时间(s)、容积CT剂量指数(CT dose index, CTDIvol)、剂量长度乘积(dose length product, DLP)。测量肝脏、脾脏、门静脉CT值,由两位医师对图像质量进行5分制评估。结果:2组图像客观评估差异均无统计学意义(P>0.05),主观图像质量评分均≥3分,图像质量均满足临床诊断要求,图像密度评分A组5分56例,4分3例,B组5分46例,4分10例,3分4例,具有统计学意义(P<0.05);2组间扫描时间、CTDIvol、DLP差异无统计学意义(P>0.05),mAs差异具有统计学意义(P<0.05)。结论:100KV结合自动管电流调制技术、固定SD值时所获门静脉图像质量能够满足诊断要求,同时辐射剂量无增加。Objective:To explore the application of 320-row volume CT with low tube voltage combined with automatic tube current modulation technique and standard deviation(SD) control portal vein imaging, evaluate its effect on image quality and feasibility of reducing radiation dose.Methods:Continuous selection 119 cases of abdomen CT examination were randomly divided into two groups: A and B,automatic tube current modulation scanning technology was used in both groups, respectively using 100 kv(A group) 120 kv(B group).The preset standard deviation(SD) of both groups was 7.5, record 2 group of patients with portal period milliampere second(mAs), scan time(s), CT volume dose index(CTDIvol), dose length product(DLP). the CT values of liver, spleen, portal vein were measured and Image quality was assessed on a 5-point scale by 2 physician.Results:There was no statistically significant difference in objective assessment between the two groups(P>0.05).The subjective image quality scores ≥3, and the image quality met the requirements of clinical diagnosis. The image density score : 56 cases with 5 points and 3 cases with 4 points in group A, 46 cases with 5 points, 10 cases with 4 points, and 4 cases with 3 points in group B(P<0.05). And between the 2 groups.There was no statistically significant difference in scan time, CTDIvol, DLP(P>0.05), and the difference in mAs was statistically significant(P<0.05).Conclusion:The portal vein image quality can be satisfied diagnostic requirements when 100 KV combined with automatic tube current modulation technology and fixed SD value and no increase in radiation dose.
