经阴道四维子宫输卵管超声造影诊断输卵管通而不畅的应用分析

摘　　要：目的:探讨经阴道四维子宫输卵管超声造影诊断输卵管通而不畅的应用价值。方法:选取2017年10月-2020年1月于我院就诊的85例疑似输卵管通而不畅患者,所有患者均予以经阴道四维子宫输卵管超声造影检查,"金标准"选用腹腔镜下通液术结果,并分析经阴道四维子宫输卵管超声造影诊断输卵管通而不畅的价值。结果:经腹腔镜下通液术检查结果得知,入选85例疑似输卵管通而不畅患者中15例诊断为阳性,占17.65%,70例为阴性,占82.35%;经阴道四维子宫输卵管超声造影检查的准确度为95.29%,灵敏度为93.33%,特异度为95.71%,阳性预测值为82.35%,阴性预测为98.53%;且经阴道四维子宫输卵管超声造影诊断与腹腔镜下通液术结果具有极好的一致性(Kappa=0.846)。结论:输卵管通而不畅采用经阴道四维子宫输卵管超声造影具有较高的准确度、灵敏度及特异度,诊断价值较高,可为临床诊断与治疗输卵管通而不畅疾病提供可靠依据,值得临床推广与应用。Objective:To explore the application value of transvaginal four-dimensional hysterosalpingography contrast in the diagnosis of partial tubal obstruction.Methods:Eighty-five patients with suspected partial tubal obstruction who were treated in the Hospital from October 2017 to January 2020 were enrolled. All patients underwent transvaginal four-dimensional hysterosalpingography contrast. With the results of laparoscopic hydrotubation as "Gold Standard", the value of transvaginal four-dimensional hysterosalpingography contrast in the diagnosis of partial tubal obstruction was evaluated.Results:The results of laparoscopic hydrotubation revealed that, out of the 85 patients with suspected partial tubal obstruction, 15 were positive, accounting for 17.65%, and 70 were negative, accounting for 82.35%. The accuracy, sensitivity, specificity and the positive predictive value and negative predictive value of transvaginal four-dimensional hysterosalpingography contrast were 95.29%, 93.33%, 95.71%, 82.35%, and 98.53% respectively. The results of transvaginal four-dimensional ultrasound hysterosalpingography contrast diagnosis and laparoscopic hydrotubation showed excellent consistency(Kappa=0.846).Conclusion:The application of transvaginal four-dimensional hysterosalpingography contrast is with high accuracy, sensitivity and specificity and value in the diagnosis of partial tubal obstruction. It can provide a reliable basis for the clinical diagnosis and treatment of partial tubal obstruction;therefore, it is with significant value to be popularized in clinical applications.
