摘 要:目的:探讨超声心动图评价青年急性心肌梗死患者右室功能的应用价值。方法:分析95例成功行经皮冠状动脉介入治疗(PCI)的青年心肌梗死患者,分为下壁合并右室心梗组(25例)、下壁非合并右室心梗组(30例)、前壁心梗组(40例),另选取30例正常对照组,分别予以三维超声心动图检查,记录并比较右室功能相关评价指标。结果:与正常对照组比较,下壁非合并右室心梗组和下壁合并右室心梗组患者的右室射血分数(RVEF)明显降低(P<0.05),而右室收缩末期容积(RVESV)明显升高(P<0.05);与正常对照组比较,下壁合并右室心梗组患者的右室舒张末期容积(RVEDV)明显升高(P<0.05);与正常对照组比较,下壁合并右室心梗组、下壁非合并右室心梗组及前壁心梗组三尖瓣环与右室游离壁处的Sm及心房收缩期峰值运动速度(Am)均明显降低(P<0.05);与正常对照组比较,下壁非合并右室心梗组及前壁心梗组三尖瓣环与室间隔结合处的心房舒张早期峰值运动速度(Em)明显降低(P﹤0.05)。结论:超声心动图多种参数可评价青年急性心肌梗死患者的右室功能异常,是简单、方便评价右室功能的影像学方法。Objective:To investigate the value of echocardiography in evaluating right ventricular function in young patients with acute myocardial infarction. Methods:Ninety-five young patients with myocardial infarction who underwent successful percutaneous coronary intervention(PCI) were analyzed and divided into inferior wall with right ventricular myocardial infarction group(25 cases), inferior wall without right ventricular myocardial infarction group(30 cases), and anterior wall myocardial infarction group(40 cases), and another 30 normal controls were selected and underwent three-dimensional echocardiography, and the right ventricular function-related evaluation indicators were recorded and compared. Results:Compared with the normal control group, the right ventricular ejection fraction(RVEF) was significantly decreased(P>0.05), while the right ventricular end systolic volume(RVESV) was significantly increased(P>0.05) in the inferior wall without right ventricular myocardial infarction combined with right ventricular myocardial infarction group;compared with the normal control group, the right ventricular end diastolic volume(RVEDV) was significantly increased(P>0.05) in the inferior wall with right ventricular myocardial infarction group, and the Sm and peak atrial systolic velocity(Am) at the tricuspid annulus and right ventricular free wall were significantly decreased(P>0.05) in the inferior wall without right ventricular myocardial infarction group;compared with the normal control group, the peak atrial diastolic velocity(Em) at the junction of tricuspid annulus and interventricular septum was significantly decreased(P>0.05) in the inferior wall without right ventricular infarction group. Conclusion:Echocardiography with multiple parameters can evaluate right ventricular dysfunction in young patients with acute myocardial infarction and is a simple and convenient imaging method for evaluating right ventricular function.
