PV患者外周血T细胞亚群、IL-17A和IL-23表达及临床意义

摘　　要：目的探究寻常型银屑病(PV)患者外周血T细胞亚群结构、白细胞介素-17A(IL-17A)和白细胞介素-23(IL-23)表达的临床意义。方法选取2018年4月至2020年3月入院治疗的银屑病患者114例作为PV组,选取60例本院同期健康体检者作为对照组。比较两组外周血T细胞亚群白细胞分化抗原3(CD3^(+))、白细胞分化抗原4(CD4^(+))、白细胞分化抗原8(CD8^(+))、CD4^(+)/CD8^(+)及炎症因子IL-17A、IL-23、白细胞介素-6(IL-6)、肿瘤坏死因子α(TNF-α)表达水平,比较不同严重程度PV患者T细胞亚群结构及炎症因子水平;对PV患者治疗前后上述指标进行比较;分析PV患者T细胞亚群与炎症因子的相关性。结果PV组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)明显低于对照组,CD8^(+)明显高于对照组,差异均有统计学意义(P<0.05)。PV组IL-17A、IL-23、IL-6、TNF-α水平明显高于对照组,差异均有统计学意义(P<0.05)。PV患者CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平:轻度组>中度组>重度组,CD8^(+)、IL-17A、IL-23、IL-6、TNF-α水平:轻度组<中度组<重度组,差异均有统计学意义(P<0.05)。与治疗前相比,PV组治疗后CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平明显上升,CD8^(+)、IL-17A、IL-23、IL-6、TNF-α水平明显下降,差异均有统计学意义(P<0.05)。PV患者CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平与IL-17A、IL-23、IL-6、TNF-α呈负相关(P<0.05),CD8^(+)水平与IL-17A、IL-23、IL-6、TNF-α水平呈正相关(P<0.05)。结论PV患者外周血T细胞亚群结构及炎症因子的检测对于病情程度评估诊断具有一定的意义。Objective To explore the structure of peripheral blood T cell subsets,and clinical significance of interleukin-17 A(IL-17 A)and interleukin-23(IL-23)in patients with psoriasis vulgaris(PV).Methods A total of 114 PV patients treated in the hospital from April 2018 to March 2020 were enrolled as the PV group,while another 60 healthy controls during the same period were enrolled as the control group.The expression levels of peripheral blood T cell subsets[cluster of differentiation 3(CD3^(+)),cluster of differentiation 4(CD4^(+)),cluster of differentiation 8(CD8^(+)),CD4^(+)/CD8^(+)]and inflammatory factors[IL-17 A,IL-23,interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)]were compared between the two groups.The structure of T cell subsets and levels of inflammatory factors among PV patients with different severity were compared.The above indicators in PV patients before and after treatment were compared.The correlation between T cell subsets and inflammatory factors was analyzed.Results CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the PV group were significantly lower than those in the control group,while CD8^(+)was significantly higher than that in the control group(P<0.05).The levels of IL-17 A,IL-23,IL-6 and TNF-αin the PV group were significantly higher than those in the control group(P<0.05).The levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)were the highest in the mild group,followed by the moderate group and the severe group.The levels of CD8^(+),IL-17 A,IL-23,IL-6 and TNF-αwere the highest in the severe group,followed by the moderate group and the mild group(P<0.05).After treatment,levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)were significantly increased,while the levels of CD8^(+),IL-17 A,IL-23,IL-6 and TNF-αwere significantly decreased in the PV group(P<0.05).The levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)were negatively correlated with IL-17 A,IL-23,IL-6 and TNF-α,while CD8^(+)level was positively correlated with them(P<0.05).Conclusion The detection of T cell subgroup structure and inflammatory factors in per
