内耳MRI造影在梅尼埃病诊断中的应用及影响因素

摘　　要：目的探讨内耳3D-FLAIR序列MRI造影在梅尼埃病诊断中的应用价值,并对其影响诊断因素进行分析。方法选取2016年12月~2019年5月我院确诊梅尼埃病患者25例共32侧患耳做为研究对象,经鼓室注射钆喷酸葡胺对比剂24h后进行内耳MRI检查,分析前庭及耳蜗膜迷路积水情况。结果在25例梅尼埃病患者32侧患耳中,26耳(81.2%)在前庭观察到内淋巴积水,3耳(9.4%)未观察到积水,另外3耳(9.4%)不能确定;在耳蜗观察到内淋巴积水18耳(56.25%),6耳(18.75%)未观察到积水,另外8耳(25%)不能确定。结论内耳3D-FLAIR序列MRI造影在前庭、耳蜗内淋巴积水中阳性率高,同时可以对积水程度分级。主要相关影响因素包括造圆窗膜透过不良、鼓室给药量、MRI检查技术及患者发病时间距离磁共振检查时间。Objective To investigate the value of 3D-FLAIR sequence MRI in the diagnosis of Meniere's disease,and to analyze the diagnostic factors of this disorder.Methods Thirty-two ears of 25 patients with Meniere's disease diagnosed in our hospital from December 2016 to May 2019 were selected as the subjects of study.MRI of the inner ear was performed 24 hours after tympanic injection of meglumine gadolinium acid contrast agent.The situation of labyrinth hydrops in the vestibule and cochlear membrane was analyzed.Results Among the 32 ears of 25 patients with Meniere's disease,endolymphoid hydrops were found in 26 ears(81.2%),endolymphoid hydrops were not found in 3 ears(9.4%),hydrops were not found in 3 cases(9.4%),hydrops were found in 18 ears(56.25%),6 ears(18.75%)were not found in cochlea,and 8 ears(25%)were not sure.Conclusion The positive rate of the 3-D-FLAIR sequence in the inner ear is high in the vestibule and in the inside of the cochlea,and the degree of water accumulation can be graded.The main influencing factors include the transmission of the film of the round window,the dose of the tympanum,the detection technique of the MRI,and the time of the patient's time of onset of the magnetic resonance.
