三血管(3-VV)和三血管气管切面(3-VT)检查在超声产前胎儿心脏大血管畸形诊断中的应用价值研究

摘　　要：目的:三血管(3-VV)和三血管气管切面(3-VT)检查在超声产前胎儿心脏大血管畸形诊断中的应用价值研究。方法:选取了2018年12月—2020年6月在我院检查中发现的胎儿心脏大血管畸形的孕妇100例,对孕妇又进行三血管及三血管切面检查,观察并分析其图形特征。结果:统计发现胎儿采用三血管及三血管切面检查的检出率为92.8%,发生主动脉弓缩窄、主动脉弓离断、双主动脉弓、右位主动脉弓/右位导管、右位主动脉弓/左位导管、永存动脉干、右室双出口、肺动脉狭窄、永存左上腔静脉、大动脉转位、法洛四联症、动脉导管迂曲数量分别为5、8、13、20、15、10、24、22、15、42、13、19例;血流方向、血管数量、血管位置常、血管管径大小异常分别为18、22、25、43例。CDFI表现为“oVo”、“Ioo”、“U”、“C”、“O”“Ⅴ”型。结论:三血管及三血管切面检查对于胎儿心脏大血管畸形有着较高的检出率,能够较为准确的发现患儿发病的类型,血管状况,对于临床诊断有着较高的意义,可在临床上推广应用。Objective To study the application value of three-vessel(3-VV)and three-vessel tracheal section(3-VT)in the diagnosis of prenatal ultrasound for fetal heart and large vessel malformations.Methods A total of 100 pregnant women with fetal cardio-macrovascular malformations found in our hospital from December 2018 to June 2020 were selected.The pregnant women were subjected to three-vessel and three-vessel section examinations to observe and analyze their graphic characteristics.Results Statistics showed that the detection rate of the fetus using three-vessel and three-vessel cross-sectional examination was 92.8%,and the occurrence of aortic arch constriction,aortic arch interruption,double aortic arch,right aortic arch/right catheter,right aortic arch/left catheter,and permanent artery The numbers of trunk,double outlet of right ventricle,pulmonary artery stenosis,permanent left superior vena cava,transposition of the great arteries,tetralogy of Fallot,and tortuosity of arterial duct are 5,8,13,20,15,10,24,22,15,42,13,19 cases respectively;Blood flow direction,number of blood vessels,blood vessel location,abnormal blood vessel diameter were 18,22,25,43 cases.CDFI is represented as“oVo”,“Ioo”,“U”,“C”,“O”and“Ⅴ”.Conclusion Three-vessel and threevessel cross-sectional examination have a high detection rate for fetal heart and large vascular malformations,and can more accurately find the type of disease and vascular condition in children.It is of high significance for clinical diagnosis and can be promoted in the clinic.application.
