摘  要:痛风炎症急性发作与中性粒细胞、巨噬细胞、核苷酸结合寡聚化结构域样受体蛋白3 (NLRP3)炎症小体及嘌呤受体2(P2) X7受体等识别单钠尿酸盐(MSU)结晶,激活NLRP3及P2X7受体等信号通路,释放炎症介质,促使局部白细胞介素-1β水平升高,诱导急性炎症反应发作相关。急性炎症通常两周后自行消散,此过程与髓样分化因子88介导的Toll样受体信号通路的激活、抗炎转化生长因子-β1的释放、中性粒细胞形成NETosis途径的阻断及中性粒细胞产生聚集性中性粒细胞胞外诱捕网包裹MSU结晶密切相关。对痛风急性发作与自行消散相关机制及其作用靶点和特点的进一步研究将为后续临床研究提供参考。Acute attack of gout inflammation is associated with neutrophils,macrophages,nucleotide-binding oligomerization domain-like receptor protein 3( NLRP3) inflammasome and purine receptor 2( P2) X7 receptor recognizing monosodium urate( MSU) crystals,activating NLRP3 and P2 X7 receptor signaling pathways,releasing inflammatory mediators,increasing local interleukin-1β level and inducing acute inflammation. Acute inflammation usually dissipates spontaneously after two weeks. This process is closely related to the activation of Toll-like receptor signaling pathway mediated by myeloid differentiation factor 88,the release of anti-inflammatory transforming growth factor-β1,the blocking of NETosis pathway of neutrophils and the production of aggregated neutrophil extracellular trapping nets encapsulated MSU crystals. Further study on the mechanism,targets and characteristics of acute attack and spontaneous dissipation of gout will provide reference for the following clinical research.
