摘　　要：目的研究艾司唑仑干预单侧初次全髋关节置换术后患者疼痛及术后早期康复的影响。方法选取91例接受全款关节置换术的患者,随机分为安慰剂组(n=45)和试验组(n=46),试验组分别于术前及术后8 d内给与2 mg艾司唑仑片口服,安慰剂组给予患者同剂型剂量安慰剂口服,收集患者基线数据、匹兹堡睡眠质量指数(PSQI)、术后疼痛视觉模拟评分(VAS)、髋关节Harris评分及术后患者第1、4、8天实验室检查结果,评估患者院内早期康复情况。结果试验组术后第4、8天PSQI低于安慰剂组(P<0.05),试验组患者术后疼痛及髋关节功能均较安慰剂组显著改善(P<0.05),术后第8天,试验组患者血红蛋白及白蛋白显著高于安慰剂组(P<0.05),术后第1、4天均未发现明显改善(P>0.05),试验组术后第1、4、8天超敏C反应蛋白低于安慰剂组(P<0.05)。结论通过术前及术后给予患者口服8天艾司唑仑可以显著改善患者院内早期康复状态。Objective To investigate the effects of estazolam on the pain and early rehabilitation of patients after total hip replacement arthroplasty.Methods A total of 91 patients who underwent total hip replacement were randomly divided into placebo group(n=45)and experimental group(n=46).The patients in observation group were treated by oral estazolam(2mg)within 8 days before and after operation,however the patients in placebo group were treated by the same dosage placebo.The baseline data,Pittsburgh Sleep Quality Index(PSQI),Pain Visual Analogue Scale(VAS)after operation,Harris hip joint function score and the laboratory examination results at 1d,4d,8d after operation,and the early recovery situation were observed and compared between the two groups.Results The PSQI at 4d and 8d in experimental group were significantly lower than those in placebo group(P<0.05),the postoperation pain and hip joint function in experimental group were superior to those in placebo group(P<0.05).At 8d after operation,the levels of hemoglobin and albumin in experimental group were significantly higher than those in placebo group(P<0.05),however there were no significant differences at 1d and 4d after operation between the two groups(P<0.05).Moreover the levels of hypersensitivity C reaction protein at 1d,4d,and 8d after operation in experimental group were significantly lower than those in placebo group(P<0.05).Conclusion To give oral estazolam for 8 days to the patients after total hip replacement arthroplasty can significantly improve the early rehabilitation status of patients.
