摘　　要：目的研究左乙拉西坦联合丙戊酸钠治疗癫痫的临床疗效及对脑损伤标志物和炎性因子的影响。方法168例癫痫患者随机分为试验组(n=90)和对照组(n=78)。试验组患者予左乙拉西坦联合丙戊酸钠治疗,对照组予丙戊酸钠治疗。治疗6个月后评价2组癫痫患者临床疗效及脑损伤标志物血清S-100β、高迁移率族蛋白-1(HMGB-1)和炎性因子白介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)的变化。结果治疗后,试验组总有效率高于对照组(P<0.05)。治疗后,2组患者血清S-100β、HMGB-1、IL-6、TNF-α水平与治疗前比较有显著下降,差异均有统计学意义(P<0.05);且试验组治疗后血清S-100β、HMGB-1、IL-6、TNF-α水平显著低于对照组(P<0.05)。结论左乙拉西坦联合丙戊酸钠治疗癫痫的临床疗效显著优于丙戊酸钠单药治疗,且脑损伤标志物血清S-100β、HMGB-1和炎性因子IL-6和TNF-α水平降低更显著,具有一定的临床应用价值。Objective To investigate the Therapeutic effects of levetiracetam combined with sodium valproate on epilepsy and their influence on brain injury markers and inflammatory cytokines of patients.Mehthods A total of 168 patients with epilepsy were randomly divided into experimental group(n=90)and control group(n=78).The patients in experimental group were treated by levetiracetam combined with valproate,while those in control group were treated by valproate.After 6-month treatment,the clinical effects and the changes of brain injury markers-S100,HMGB1,IL-6 and TNF-αwere observed and compared between the two groups.in both groups were evaluated.Results After treatment,the total effective rate in experimental group was significantly higher than that in control group(P<0.05).The levels of S100β,HMGB1,IL-6 and TNF-αin both groups were significantly decreased(P<0.05),moreover,which in experimental group were significantly lower than those in control group(P<0.05).Conclusion The therapeutic effects of levetiracetam combined with sodium valproate are superior to those of sodium valproate alone in treatment of epilepsy,moreover,the serum levels of brain injury markers S100β,HMGB1 as well as inflammatory factors IL-6 and TNF-αare decreased significantly,which is of a certain clinical application value.
