血脉通方预防后路脊柱手术后深静脉血栓形成的疗效及对血液流变学的影响

摘　　要：目的讨论血脉通预防后路脊柱术后下肢深静脉血栓形成效果及对血液流变学的影响。方法收治的行后路脊柱手术后患者120例,按随机数字表法分为治疗组及对照组,每组60例。2组患者术后均进行血脂控制、戒酒及戒烟等基础预防措施,对照组采用低分子肝素钙治疗,皮下注射4100 U,1次/d,持续治疗7 d。治疗组于术后连续7 d应用低分子肝素钙的基础上服用血脉通治疗,1剂/d。分析术后3 d及术后14 d 2组下肢深静脉血栓的发生率,双下肢大,小腿周径测量差值,血液流变学指标,凝血指标。结果治疗组14 d后下肢深静脉血栓的发生率为1.67%低于对照组5.00%(P<0.05);2组术后不同时间段大、小腿周径差均值比较差异无统计学意义(P>0.05)。2组术后3 d、14 d血液流变学指标及凝血指标较术后1 d明显改善(P<0.05);治疗组术后14 d血液流变学指标、凝血指标均好于对照组,差异有统计学意义(P<0.05)。结论血脉通对预防脊柱术后下肢深静脉血栓有效可减轻术后血液高凝状态,并对术后血液循环动力起到改善作用,值得临床推广。Objective To investigate the efficiency of Xuemaitong decoction in preventing deep venous thrombosis after posterior spinal surgery and its influence on hemorheology.Methods A total of 120 patients who were treated by posterior spinal surgery in our hospital were enrolled in the study,who were divided into observation group and control group according to random number table,with 60 cases in each group.The patients in both groups were given basic treatment including controlling of blood lipid level and weight,adequate exercise,smoking and drinking cessation.On that basis,the patients in control group were treated by subcutaneous injection of low-molecular weight heparin calcium 4,100U,once a day,for 7 days,however,the patients in observation group,on the basis of control group,were treated by Xuemaitong decoction,once a day for 7 days.The incidence rate of deep venous thrombosis,and the levels of hemorheology indexes,coagulation indexes,as well as the deviation of both lower extremities circumference at 1d,3d,14d after surgery were observed and compared between the two groups.Results The incidence rate of deep venous thrombosis at 14d after surgery in observation group was 1.67%,which was significantly lower than that(5.00%)in control group(P<0.05).There were no significant differnces in the deviation mean values of both lower extremities circumference in different time points between the two groups(P>0.05).The hemorheology indexes and coagulation indexes at 3d,14d after surgery in both groups were significantly improved,moreover,which at 14d after surgery in observation group were superior to those in control group(P<0.05).Conclusion Xuemaitong decoction is effective in preventing the incidence of deep venous thrombosis after lumbar surgery,which can relieve the hypercoagulability,improve the blood circulation,therefor which is worthy of clinical promotion.
